Hi to all.

Would someone be kind enough to outline the procedure where urea 40% is used to remove the nail plate?

I ask in relation to enhancing local drug delivery to a 9 year old child with a proven fungal infection, where topical treatment and frequent nail debridement has been ineffective.

You use occlusive dressings over the nail with urea. Keep covered and replace every 2-4 days. Usually with 14 to 21 days the nails sheds. Watch out for skin reaction though as this can arise in sensitive skins (urticaria).

Foot note - shedding the nail may not assist in the treatment of fungal infection as the infection site is invariably the matrix. Maureen O'Donnell did some work using nail avulsions and topical antifungal agents some time ago. No too sure of the final outcomes but seem to remember it was not 100% successful.
