Effectiveness of negative pressure wound therapy in treating diabetic foot ulcers: a systematic review and meta-analysis of randomized controlled trials

Background: Diabetic foot ulcers (DFUs) present devastating complications. Management of these wounds includes applying a vacuum device composed of a fenestrated tube embedded in a foam dressing and connected to a vacuum pump to collect the fluid in a vessel.

Objective: To review the effectiveness of negative pressure wound therapy (NPWT) in treating DFUs and evaluate the clinical outcomes of full and partial wound healing.

Methods: A systematic review and meta-analysis of randomized controlled trials was conducted after searching the PubMed, Web of Science, and Cochrane Library databases. Studies that described the use of NPWT in the management of DFUs were included and compared with different interventions for the proposed outcomes.

Results: A total of 14 studies were included, 1 of which was later excluded during internal validation. Wound healing outcomes were analyzed in 2 studies, showing the superiority of total wound healing in the NPWT group. Ten studies compared the effect of different interventions on wound area reduction and 8 of these results were best with NPWT. Three studies showed clinical and statistical homogeneity (I² = 100%), making it possible to conduct a meta-analysis.

Conclusion: Although the articles included in this systematic review have limitations regarding methodological quality and clinical heterogeneity, the results showed the potential benefit of NPWT in healing and wound area reduction. These outcomes are essential in the prevention of amputation in patients with DFU.

Combination therapy of negative pressure wound therapy and antibiotic-loaded bone cement for accelerating diabetic foot ulcer healing: A prospective randomised controlled trial

Negative pressure wound therapy (NPWT) and antibiotic-loaded bone cement (ALBC) are commonly used treatments for diabetic foot ulcers (DFUs). However, the combined efficacy of these two modalities remains unclear. This clinical study aimed to assess the effectiveness and underlying mechanisms of NPWT&ALBC in the management of DFUs. A total of 28 patients were recruited, 16 of whom served as controls and received only NPWT, whilst 12 received NPWT&ALBC. Both groups underwent wound repair surgery following the treatments. Blood samples were obtained to detect infections and inflammation. Wound tissue samples were also collected before and after the intervention to observe changes in inflammation, vascular structure and collagen through tissue staining. Compared with the NPWT group, the NPWT&ALBC group exhibited a superior wound bed, which was characterised by reduced inflammation infiltration and enhanced collagen expression. Immunostaining revealed a decrease in IL-6 levels and an increase in α-SMA, CD68, CD206 and collagen I expression. Western blot analysis demonstrated that NPWT&ALBC led to a decrease in inflammation levels and an increase in vascularization and collagen content. NPWT&ALBC therapy tends to form a wound bed with increased vascularization and M2 macrophage polarisation, which may contribute to DFUs wound healing.

Amputation rates have dropped and diabetes awareness has increased due to several diabetic foot treatment suggestions. Diabetic foot ulcers (DFUs) may now be properly treated with a range of cutting-edge medicines, allowing practitioners the greatest local therapy available. One is negative pressure wound therapy. Aim of work: Assessment of vacuum assisted closure clinical efficacy compared with the standard moist wound dressing in the treatment of diabetic foot ulcers. Patients and methods: A total of 30 diabetic patients with diabetic foot ulcers were enrolled, after consenting each of them and divided into two groups; group A (standard moist wound dressing) (SMWD) (control group) included fifteen cases who were treated with conventional moist dressing and group B (vacuum assisted Closure) (NPWT) (experimental group) included fifteen cases who were treated vacuum assisted closure. Progress of healing was evaluated and documented in the form of change in wound diameter, depth, up or down scaling along University of Texas wound classification (UTWC), wound status at 2, 4, 8, and 12 weeks and 4 weekly thereafter till complete epithelialization with diligent recording of time required for complete epithelisation and number of dressings required in the process for both groups. Results: Vacuum assisted wound closure is associated with better healing and diminished ulcer depth after 8 weeks of treatment with amount of change 100.00 ± 0.00 vs. 93.79 ± 5.55%. It is also associated with higher incidence of complete granulation after 6 weeks of treatment 12 (80.0%) vs. 6 (40.0%). There are no differences were noted between study groups as regard age, sex., special habits and medical co morbidities, laboratory investigations, ulcer site, ucler grading regarding University of Texas before and after treatment. Conclusion: VAC is safer and more effective than moist dressing for diabetic foot ulcers. VAC therapy speeds wound healing, accelerates granulation tissue production, and reduces ulcer area compared to standard dressing.
