Hi there,

I just saw a DVA patient (age 34, male) with recent R/AKA and pain of the rest of foot.
He started running, and concern that the left shoe worn very quickly.

What I did today, bio assessment and cast for L/foot - the ROM of STJ and MTJ was WIN, mild gastroc eqnius. I will make an orthotics with simple accommodate orthotics to distribute pressure and control excessive pronation, and gave instruction of gastroc stretching.

I would appreciate if you could give me any advise for one foot prescription.

Also since he does not any deformity on his L/foot, and he wants to run, does he need to purchase of joggers by himself? Does somebody know any support system of DVA (Australia) for him?

a couple of questions

What kind of prosthesis is he running in? Is it his everyday leg or an athletic leg
What distances is he running? Have there been any failures to the prosthesis?
How is his gait, walking and running?
I would have a discussion with his prosthotist, to see if some mods to the leg may be required.
Also don't think DVA will fund footwear, and in any case they only fund specific brands, which don't include any running shoes.

He has 2 different types - both made through defence, and not local hospital.
He shows video of running - with carbon fiber (the J shape) one (so not with shoe). Daily activity - prastic one (with shoe).

His gait - the clinic is not set up for check running - so only checked walking though -
and his artificial foot was in shoe and other foot was bearfoot, so difficult to check the normal gait as LLD compensation.

He came with 2-3/52 old joggers today, so did not notice any abnormal worn but reported L-R uneven worn (as running he does only wear for L).

For orthotics - I thought it is better to get stronger polly shell and extra modification for stability (med and lat flare).

I am not sure if this is a current policy for DVA, however 10 yrs ago the policy was that, if the patient has DVA entitlements and also an amputee -- DVA would pay the price of one pair of footwear as part of the prostheses entitlement. The patient would purchase their own footwear and would be reimbursed by DVA upon receipt presentation.
The patient's "remaining limb" condition was not a consideration for reimbursement.
Best to check with your State's DVA Podiatry Advisor.

Just FYI what I found today through DVA (but please re-check by yourself as it may be changed later)

DVA gold card holder with AKA (I believe BKA would apply same), can have financial support upto $200/pair of shoes. The patient claim to DVA with invoice and letter from podiatrist.
