Higher body mass index is associated with plantar fasciopathy/plantar fasciitis: systematic review and meta-analysis of various clinical and imaging risk factors

Question What (risk) factors are associated with plantar fasciopathy (PF)?

Design Systematic review with meta-analyses.

Participants Patients with PF.

Factors All factors described in prospective, case?control or cross-sectional observational studies.

Results 51 included studies (1 prospective, 46 case?control and 4 cross-sectional studies) evaluated a total of 104 variables. Pooling was possible for 12 variables. Higher body mass index (BMI) (BMI>27, OR 3.7 (95% CI 2.93 to 5.62)) in patients with PF was the only significant clinical association, and its effect was the strongest in the non-athletic subgroup. In people with PF compared to controls, pooled imaging data demonstrated a significantly thicker, hypoechogenic plantar fascia with increased vascular signal and perifascial fluid collection. In addition, people with PF were more likely to have a thicker loaded and unloaded heel fat pat, and bone findings, including a subcalcaneal spur and increased Tc-99 uptake. No significant difference was found in the extension of the first metatarsophalangeal joint.

Conclusions We found a consistent clinical association between higher BMI and plantar fasciopathy. This association may differ between athletic and non-athletic subgroups. While consistent evidence supports a range of bone and soft tissue abnormalities, there is lack of evidence for the dogma of clinical and mechanical measures of foot and ankle function. Clinicians can use this information in shared decision-making.

Effect of Surgical Weight Loss on Plantar Fasciitis and Healthcare Utilization.

Background: Plantar fasciitis (PF) is known to be one of the most common causes of heel pain. Obesity is recognized as a significant factor in PF development, possibly due to increased mechanical loading of the foot as a result of excess weight. The benefit of bariatric surgery is documented for other comorbidities but not for PF.

Methods: Retrospective chart review of patients with a diagnosis of PF identified from a prospectively maintained database of the Cleveland Clinic Bariatric and Metabolic Institute. Age, gender, surgery, excess weight loss (EWL), body mass index (BMI), and healthcare utilization related to PF treatment were abstracted. Comparative analyses were stratified by surgery type.

Results: Two hundred twenty-eight (2.2%) from a total of 10,305 patients in our comprehensive bariatric program had a documented diagnosis of PF, of whom 163 underwent bariatric surgery and were included in the analysis. Eighty-five percent of patients were females, age was 52.2 ± 9.9 years, and, pre-intervention BMI was 45 ± 7.7. Postoperatively BMI, and %EWL was 34.8 ± 7.8 kg/m2 and 51.0 ± 20.4%, respectively. One hundred and forty-six (90%) patients achieved resolution of PF and related symptoms. Mean number of treatment modalities used for PF in patients preoperatively was 1.9 ± 1.0 (p=0.25) Post surgery, the number treatment modalities used per patient was reduced to 0.3 ± 0.1 (p=0.01).

Conclusion: We present new evidence suggesting that reductions in BMI after bariatric surgery may be associated with decreasing the numbers of visits for PF, and contribute to symptomatic improvement.

Introduction: Planter fasciitis is one of the most common causes of heel pain. The objective of this
study is to study the sonographic appearance of plantar fascia in clinically suspected cases and to
establish the correlation between plantar fasciitis, body mass index (BMI) and heel pad thickness
(HPT).
Methods: In this case controlled analytical study, we sonographically evaluated 100 patients with
clinical plantar fasciitis (unilateral: 90, bilateral: 10 with mean age 46.9yrs) and control group of 60 (120
heels) healthy volunteers with mean age 45.3yrs.
Results: Mean Plantar Fascia Thickness (PFT) and Heel Pad Thickness (HPT) are greater on the
symptomatic side for patients with unilateral and bilateral PFs than on the asymptomatic with unilateral
PFs, and also control subjects (PFT values are 4.41 ± 0.59, 4.63 ± 0.55, 2.83 ± 0.36, 2.62
± 0.37 mm, and HPT values 17.64 ± 1.07, 17.28 ± 1.10, 16.91 ± 1.06, 16.73 ± 1.13 mm, respectively).
(p <0.0001). Mean BMI values of the case and control groups are 26.14 ± 1.9 and 24.42 ± 0.89 Kg/m2
,
respectively (p <0.05). We found hypoechogenicity of plantar fascia in 80 cases (72.7%), calcaneal spur
in 69 cases (62.7%), biconvexity in 11 cases (10%) and perifascial fluid in 16 cases (14.5%) withinthe
plantar fasciitis group (110 symptomatic heels).
Conclusion: Increased plantar fascia thickness, increased heel pad thickness and hypoechogenicity
of plantar fascia are consistent sonographic findings in plantar fasciitis. Its occurrence has significant
relation to high BMI.

Chronic plantar heel pain modifies associations of ankle plantarflexor strength and body mass index with calcaneal bone density and microarchitecture

Chronic plantar heel pain (CPHP) is associated with calcaneal bone spurs, but its associations with other calcaneal bone features are unknown. This study therefore aimed to determine associations between having CPHP and bone density and microarchitecture of the calcaneus. We assessed 220 participants with CPHP and 100 age- and sex-matched population-based controls. Trabecular bone density, thickness, separation and number, BV/TV, and cortical density, thickness and area were measured using a Scanco Xtreme1 HR-pQCT scanner at a plantar and mid-calcaneal site. Clinical, physical activity and disease history data were also collected. Associations with bone outcomes were assessed using multivariable linear regression adjusting for age, sex, physical activity, BMI and ankle plantarflexor strength. We assessed for potential effect modification of CPHP on these covariates using interaction terms. There were univariable associations at the plantar calcaneus where higher trabecular bone density, BV/TV and thickness and lower trabecular separation were associated with CPHP. In multivariable models, having CPHP was not independently associated with any bone outcome, but modified associations of BMI and ankle plantarflexor strength with mid-calcaneal and plantar bone outcomes respectively. Beneficial associations of BMI with mid-calcaneal trabecular density (BMI-case interaction standardised X/unstandardised Y beta -10.8(mgHA/cm3) (se 4.6), thickness -0.002(mm) (se 0.001) and BV/TV -0.009(%) (se 0.004) were reduced in people with CPHP. Beneficial associations of ankle plantarflexor strength with plantar trabecular density (ankle plantarflexor strength -case interaction -11.9(mgHA/cm3) (se 4.4)), thickness -0.003(mm) (se 0.001), separation -0.003(mm) (se 0.001) and BV/TV -0.010(%) (se 0.004) were also reduced. CPHP may have consequences for calcaneal bone density and microarchitecture by modifying associations of BMI and ankle plantarflexor strength with calcaneal bone outcomes. The reasons for these case-control differences are uncertain but could include a bone response to entheseal stress, altered loading habits and/or pain mechanisms. Confirmation with longitudinal study is required.

Influence of Body Mass on Running-Induced Changes in Mechanical Properties of Plantar Fascia

Body mass is a major risk factor for plantar fasciopathy; however, evidence explaining the process between risk factors and injury development is limited. Long-distance running induces transient and site-specific reduction in plantar fascia (PF) stiffness, reflecting mechanical fatigue and microscopic damage within the tissue. As greater mechanical loads can induce greater reduction in tissue stiffness, we hypothesized that the degree of running-induced change in PF stiffness is associated with body mass. Ten long-distance male runners (age: 21 − 23 years, body mass: 55.5 ± 4.2 kg; mean ± SD) and 10 untrained men (age: 20 − 24 years, body mass: 58.4 ± 5.6 kg) ran for 10 km. Before and immediately after running, the shear wave velocity (SWV) of PF at the proximal site, which is an index of tissue stiffness, was measured using ultrasound shear wave elastography. Although the PF SWV significantly decreased after running in runners (−4.0%, p = 0.010) and untrained men (−21.9%, p < 0.001), runners exhibited smaller changes (p < 0.001). The relative changes in SWV significantly correlated with body mass in both runners (r = −0.691, p = 0.027) and untrained individuals (r = −0.723, p = 0.018). These results indicate that a larger body mass is associated with a greater reduction in PF stiffness. Our findings provide in vivo evidence of the biomechanical basis for body mass as a risk factor for plantar fasciopathy. Furthermore, group differences suggest possible factors that reduce the fatigue responses, such as adaptation enhancing the resilience of PF and running mechanics.

Background: Plantar fasciitis described as the inflammation of planter fascia, particularly hampering an individual’s
daily living activities related to ankle and foot that impact on quality of life. The study aimed to identify the association
between BMI and planter fasciitis among hospital staff in a selected hospital in Dhaka city.
Methods: A descriptive cross-sectional study.
Results: Mean±SD age of the respondents were 29.03±6.56 years. Among the hospital staff, the majority of the
respondents were nurses 31.25%, 13.02% respondents were physician and 2.07% of respondents were physiotherapists,
others were pharmacist, lab technician, administration staff, cleaner, security, information technologists and officers
from the maintenance department of selected hospital. According to BMI scale the 65% were normal weight, 23% were
overweight and only 12% were obese. In this study respondents who have only two family members, have strong
relationships with BMI (p<0.00). Besides, the duration of work has a slightly significant relation with BMI (p<0.03)
and BMI has a strong impact on level of pain (p<0.01) among respondents.
Conclusions: The recommendation for risk variables that have a strong correlation with plantar fasciitis is the working
status, duration, and BMI.

Objective: To determine the effects of body mass index and ankle dorsiflexion in patients with plantar fasciitis. Study Design: A cross-sectional. Place and Duration of Study: The study was carried out at the Department of Physical Therapy of the two leading tertiary care centers, Advance Orthopedic & Reconstructive Institute & Dr. Ziauddin Hospital Karachi, Pakistan from August 2022 to June 2023. Methods: Sixty-Four participants were recruited from the physical therapy departments of two tertiary care centers in Karachi by convenience sampling. The study focuses on the impact of demographics on the performance of plantar fasciitis patients. The measurement tools used in this are BMI (Body mass index), NRS (Numeric rating scale), LEFS (Lower extremity functional scale), and Goniometer. The data were analyzed by SPSS version 23.0. Results: Our findings show that plantar fasciitis is more common in the obese-II category with reference to BMI, females are more affected than males with reference to gender, the middle age group is more affected with reference to age and dorsiflexion angle of patients is less than normal range shows the directly proportional of demographics with the performance of patients with plantar fasciitis. Conclusion: Our study showed the positive effect of ankle dorsiflexion angle and BMI, gender, pain, and age on the physical performance of plantar fasciitis patients.
