The question should not be prefab vs custom, the question should be - what are you treating (what is the primary pathology that warrants orthotic intervention).

On many occasions when I have asked this question, the response has been - I am treating overpronation (abnormal pronation, hyperpronation etc). But overpronation is an observation, a symptom, not a primary etiology.

So until the etiology is determined, the question regarding prefab vs custom orthotics is Moot.
