  4. Ian, this is mentioned in the book: Biomechanics of Musculoskeletal Injury (1998) by Whiting and Zernicke, Human Kinetics p.171. The references they provide are:

    I have managed to get a copy of the Jozsa paper, but am struggling to get the Kujala paper. Found it here on Science Direct but my subscription level does not allow me access to the full version - anyone out there able to help?
