Is anyone experienced with the flexor tendon transfer procedure for flexible hammertoes? Do you use a dorsal approach or dorsal and plantar? Is the procedure very difficult? Higher chance of complications?

Yes, I've done hundreds of FDL transfers to the proximal phalanx, negating the deforming force. Everything is done from a dorsal approach. After resection prox phal hd, plantar longitudinal capsulotomy exposing FDL tendon, detach tendon distally, button-hole tendon, slide over prox phal shaft, & suture in place. Excellent results.

Thanks for the reply. I actually did the procedure about 7 weeks ago and used a dorsal approach, removed the prox phalanx head, but did struggle to get under (superficial to) the flexor tendon. I finally got a mosquito under it but it took 3 assistants to retract while I pulled the split ends of the flexor up and over the proximal phalanx to suture. I never have though of buttonholing it and then sliding it over the proximal phalanx, sounds like a good way to do it. At 7 weeks out, the toe is swollen and a little bit sore at the end of the day, so I told the patient to walk less. The distal phalanx is deviated laterally, but an acceptable amount.

The study ,above , states -
"To date, no studies have looked at outcomes of operative management in the geriatric population, which may be at greater risk for complications or functional compromise because of comorbidities."
Questions I would like to see asked and answered are (1) what happens to intrinsic muscle function in the geriatric population postoperatively and in particular , if significant muscle atrophy does occur following recuperation from hammer toe surgery , (2) is previous strength ever regained in these muscles ?
I do not believe that questionnaires can answer these important questions . -

That might be true of the hammer toe but what about its pals . As the repaired hammer toe is rested following surgery ,the neighboring toes are likely to become atrophied and in a geriatric patient may never recover their former strength .
However the questions I asked are unnecessarily narrow and might better be written as -
" Questions I would like to see asked and answered are (1) what happens to intrinsic muscle function in the geriatric population postoperatively and in particular , if significant muscle atrophy does occur following recuperation from any type of surgery which requires a patient to be off their feet for 3 weeks , (2) is previous strength ever regained in these muscles ? -
