Background: Diabetic foot ulcer is a worldwide health care concern affecting tens of thousands of patients. If these ulcers left untreated, they can create severe complications.

Objectives: This study was designed to examine the effect of infrared radiation on the healing of diabetic foot ulcer.

Patients and Methods: This clinical trial study was performed on 50 patients referred to Dr. Ganjavian hospital in Dezful city, Iran, with diabetic foot ulcer degree 1 and 2 (based on Wegener Scale). Sample size was determined based on relevant studies of the recent decade. Patients were classified into the intervention and control groups (n = 25 in each group) in terms of age, gender, degree of ulcer, ulcer site and body mass index. In this study, work progress was evaluated according to the checklist of diabetic foot ulcer healing evaluation.

Results: The results of the current study showed that there was a statically significant difference in healing ulcers (P < 0.05) and mean healing time (P < 0.05) between the two groups.

Conclusions: Using the infrared plus routine dressing is more effective than using merely routine dressing.

Effect of FAR Infrared Radiation on the Microcirculation of Diabetic Foot

The most frequent clinical complication is diabetics. Diabetics are characterized by elevated blood glucose levels resulting sensory nerve damage or lesions. The wounds of diabetic foot are often slow to heal require medical attention and monitoring. The purpose of this study is to evaluate the effect of intervention of infrared radiation therapy on the microcirculation of the lower extremity of diabetic foot. Ten diabetics and 4 non-diabetics were recruited in this study. Diabetics group were conducted in before and after the intervention in each month for three consecutive months; four non-diabetics group was also measured before and after the intervention, each two weeks for one month. Diabetes group: In the first month of experiment, the surface temperature of the foot was significantly improved, and there was no significant change in blood flow before and after the experiment. The second month of the experiment, the foot surface temperature did not significantly improve, but the foot of the internal blood circulation has improved significantly. In the third month of the experiment, the surface temperature of the foot was significantly decreased and the blood flow was also significantly decreased. Non-diabetics: In the first time of experiment, the surface temperature of the foot was improved but no significant change was made. There was no significant change in blood flow before and after the experiment. The second time of the experiment, the foot surface temperature did not significantly improve, but the foot of the internal blood circulation has improved significantly. In the third time of experiment, the surface temperature of the foot was significantly improved, and there was no significant difference in blood flow. The results of this study, not only on the clinical evaluation of infrared, but also can be applied to clinical products.

Diabetes is associated with many severe complications, such as heart disease and nerve damage. Diabetics have elevated blood glucose levels that result in peripheral neuropathy. As a result, diabetics always feel painful about their lower limbs and may need to have lower limb amputation. The diabetic foot ulcers are hard to heal and require extensive medical treatments and follow ups. The purpose of this study was to evaluate the effect of infrared radiation therapy using a heating pad with carbon fibers (LinkWin Technology Co., Taiwan) on the diabetic foot. Ten diabetics and 4 non-diabetics were recruited for this study. The participants were assessed for their microvascular function before and after the interventions in each month for three consecutive months. The results showed that surface temperature increase by 2° after using the pad in the first and third months (p < 0.05); and blood flow increase on the plantar foot (p < 0.05) but not on the dorsal foot. The increase in blood flow may alleviate diabetic-related complications. The results of autonomic nervous testing indicated that the activity of sympathetic nervous increased after using the pad. In conclusion, the use of heating pad with far infrared radiation could improve blood flow and autonomic nervous system in diabetics, therefore improving alleviating symptoms of diabetes-related complications.
