OBJECTIVE:
The purpose of this study was to determine the immediate effects of a single anteroposterior mobilization of the talus on the active dorsiflexion range of motion (ROM) in participants with different orthopedic foot and ankle injuries.
METHODS:
This study included 30 male and female participants aged 18 to 50 years with unilateral orthopedic foot and ankle dysfunction. All participants underwent 3 sets of active dorsiflexion ROM measurement in both ankles. Measurements included baseline, post-first treatment, and post-second treatment values. Participants received either joint mobilization or manual contact (control) on the affected ankle. Active dorsiflexion ROM was assessed using a biplanar goniometer with participants in the prone position and 90° of knee flexion.
RESULTS:
Both groups (joint mobilization and manual contact) showed increased active dorsiflexion ROM. However, the mean difference of dorsiflexion measurements before and after mobilization was greater than before and after control treatment.
CONCLUSION:
A single session of articular mobilization of the talus did not significantly increase dorsiflexion ROM in participants with orthopedic dysfunctions of the ankle and foot compared with a manual contact procedure.

The Effects of Mobilization with Movement on Dorsiflexion Range of Motion, Dynamic Balance, and Self-Reported Function in Individuals with Chronic Ankle Instability

Previous studies have examined the effectiveness of a manual therapy intervention known as Mobilization with Movement (MWM) to increase dorsiflexion range of motion (ROM) in individuals with chronic ankle instability (CAI). While a single talocrural MWM treatment has increased dorsiflexion ROM in these individuals, examining the effects of multiple treatments on dorsiflexion ROM, dynamic balance, and self-reported function would enhance the clinical application of this intervention. This study sought to determine if three treatment sessions of talocrural MWM would improve dorsiflexion ROM, Star Excursion Balance Test (SEBT) reach distances, and self-reported function using the Foot and Ankle Ability Measure (FAAM) in individuals with CAI. Eleven participants with CAI (5 Males, 6 Females, age:21.5±2.2 years, weight:83.9±15.6 kg, height:177.7±10.9 cm, Cumberland Ankle Instability Tool:17.5±4.2) volunteered in this repeated-measures study. Subjects received three MWM treatments over one week. Weight-bearing dorsiflexion ROM (cm), normalized SEBT reach distances (%), and self-reported function (%) were assessed one week before the intervention (baseline), prior to the first MWM treatment (pre-intervention), and 24-48 hours following the final treatment (post-intervention). No significant changes were identified in dorsiflexion ROM, SEBT reach distances, or the FAAM-Activities of Daily Living scale (p>0.05). Significant changes were identified on the FAAM-Sport (p=0.01). FAAM-Sport scores were significantly greater post-intervention (86.82±9.18%) compared to baseline (77.27±11.09%; p=0.01) and pre-intervention (79.82±13.45%; p=0.04). These results indicate the MWM intervention did not improve dorsiflexion ROM, dynamic balance, or patient-centered measures of activities of daily living. However, MWM did improve patient-centered measures of sport-related activities in individuals with CAI.

Background
Although gastrocnemius stretching and talocrural joint mobilization have been suggested as effective interventions to address limited ankle dorsiflexion passive range of motion (DF PROM), the effects of a combination of the two interventions have not been identified.

Objective
The aim of the present study was to compare the effects of gastrocnemius stretching combined with joint mobilization and gastrocnemius stretching alone.

Design
A randomized controlled trial.

Methods
In total, 24 individuals with limited ankle DF PROM were randomized to undergo gastrocnemius stretching combined with joint mobilization (12 feet in 12 individuals) or gastrocnemius stretching alone (12 feet in 12 individuals) for 5 minutes. Ankle kinematics during gait (time to heel-off and ankle DF before heel-off), ankle DF PROM, posterior talar glide, and displacement of the myotendinous junction (MTJ) of the gastrocnemius were assessed before and after the interventions. The groups were compared using two-way repeated measures analysis of variance.

Results/findings
: Greater increases in the time to heel-off and ankle DF before heel-off during gait and posterior talar glide were observed in the stretching combined with joint mobilization group versus the stretching alone group. Ankle DF PROM and displacement of the MTJ of the gastrocnemius were increased significantly after the interventions in both groups, with no significant difference between them.

Conclusions
These findings suggest that gastrocnemius stretching with joint mobilization needs to be considered to improve ankle kinematics during gait.

Decreased ankle dorsiflexion can be a factor that limits participation in activities and predispose individuals to chronic issues. Traditionally, stretching protocols, strengthening of muscles, balance training, as well as traditional joint mobilizations have been used to increase ankle dorsiflexion. Although current methods have been successful at mitigating ankle dorsiflexion restrictions, alternative treatments should be researched. PURPOSE: Determine possible effect of two Mulligan Mobilization with Movement Techniques on ankle dorsiflexionMETHODS: Individuals were recruited at three college athletic training clinics located around the United States. Participants were randomly allocated into two groups, the Mulligan Ankle Dorsiflexion MWM in weight bearing and the Mulligan Fibula MWM for Dorsiflexion in non-weight-bearing. Once allocated, clinicians applied a single treatment with the appropriate glides for three sets of ten. Distance from wall and Tibial angle for the weight-bearing dorsiflexion lunge test was collected pretreatment, post treatment, at a 48-hour follow up, and at a one week follow up. RESULTS: Significant effect was found for both techniques for WBLT Distance over time (F(3,33) = 29.38,ηp2=.78, p < .001), when observing pairwise comparison a statistical significance was observed (ADMWM: pretest 5.00 ± 2.75cm, posttest 7.07 ± 2.75cm, 48 hour 7.57 ±2.50cm, 1 week 7.43 ± 2.37cm; FMWM: pretest 4.50 ± 2.61cm, posttest 6.17 ± 2.38cm, 48 hour 6.50 ± 2.66cm, 1 week 6.50 ± 2.59cm). Significance was also observed WBLT Angle over time (F(3,33) = 53.6, ηp2=.88, p < .001), a statistical significance can also be observed when looking at pairwise comparison (ADMWM: pretest 30.7 ± 5.12° posttest 38.6 ± 4.35°, 48 hour 39.7 ± 3.10°, 1 week 39.1 ± 2.41°; FMWM: pretest 28.8 ± 4.22°, posttest 35.7 ± 4.27°, 48 hour 37.5 ± 5.24°, 1 week 37.8 ± 3.37°). CONCLUSIONS: The results support the use of both the Mulligan Concept techniques to increase ankle dorsiflexion ROM. The immediate increases in ankle dorsiflexion ROM were maintained at one week follow-up.

Mulligan Concept in the Treatment of Ankle Sprains

Aim of the study is to establish the efficiency of the mulligan concept vs a sham group (who will also serve as control). Patient with an subacute ankle sprains will be assess following the mulligan concept and then randomized in two groups: Mobilization with movement (MWM) group and Sham group. MWM group will receive 3 sets of 10 repetitions of treatment (i.e. passive accessory glide during an active dorsiflexion) while the sham treatment will do 3 sets of 10 active dorsiflexion with the hand of the Physiotherapist only apply on the skin. 3 sessions with 4 days apart will take place. outcomes will be measure before and after every sessions.
