Would be intrested if a undiagnosed tarsal Coaltion spotted in Mid 30's is a straight to surgery case , and what outcomes are ?

It is rare that any podiatric condition is a 'straight to surgery' situation.

In the case of congenital or acquired tarsal coalitions, the same conservative non-surgical care needs to be attempted first, as you would for most other joint disease in the foot and ankle.

Orthotics, activity modification, NSAIDs, etc, then if symptoms have not improved significantly over 3-6 months, refer for a surgical opinion.

At this age, depending on intra vs extra articular - a fusion rather than excision may be necessary.
