Worldwide, 15% of the 200 million diabetics suffer from diabetic wounds. In 1997, the cost for amputation of toes and limbs that resulted from infected diabetic foot ulcers ranged from $25000-$40000 per incident. Increasing numbers of research have shown the positive influence of pentoxifylline (PTX) on healing skin wounds. In this study, we evaluate the effect of systemic PTX (25mg/kg bid) on wound healing in 80 diabetic rats (DB) by secondary intention. Wounds (20mm×5mm) were identically inflicted on the skin area of the backs of all rats. On day 15 following surgery, a band of skin (4mm×60mm) that contained wound was extracted for biomechanical testing. For histologic analysis, both experimental (DB+PTX) and control, receiving distilled water (DB+DW) groups were further subdivided into day 3 and 7 groups. Rats were sacrificed three and seven days after surgery, and a sample from each wound was taken. All specimens were sectioned stereologically and stained with H&E. Cell counts were performed by stereological methods. Semi-quantitative evaluation of matrix metalloproteinases (MMPs) and inhibitor-1 was performed by Reversed Transcription-PCR and UVI TEC software. For statistical analysis we used student's t-test. Collectively, the results of this study demonstrate that there was significant improvement with PTX in all biomechanical parameters. Histologically, PTX reduced inflammation by day seven. Quantitatively, by day five, PTX reduced expression of MMPs and increased TIMP-1 expression. These findings revealed that PTX significantly improved wound healing indices in streptozotocin-induced DB.

Background: Diabetic foot ulcers are estimated to affect 15 % of all diabetics and precede almost 85 % of foot amputations. Pentoxyfylline a substituted xanthenes’ derivative has been reported to increase the blood flow to the microcirculation and enhances tissue oxygenation. It has been widely used in the treatment of intermittent claudication.

Materials and Methods: Pentoxyfylline is known to decrease the roulex formation of RBC & hence helps in improving the microcirculation. Out of sixty seven patients 30 received pentoxyfylline & 32 were on traditional treatment & there was loss of follow-up in five cases. The response was observed subjectively, histologically & by Doppler studies.

Results: It was observed that the patients on pentoxyfylline had early healing as compared to patients receiving only conventional treatment as evident on biopsy & Doppler.

Conclusion: Here in this research our objective was to determine whether pentoxyfylline (trental 400 mg) taken orally TDS in addition to ambulatory compression bandages & dressings improves the healing rates of diabetic ulcers.

Efficacy and Safety of Pentoxifylline for Venous Leg Ulcers: An Updated Meta-Analysis

The venous leg ulcers are debilitating, painful, and often unresponsive to advanced dressing treatments, so drugs used locally and systematically are essential adjuvant therapy-pentoxifylline (PTX) whose anti-inflammatory effects may offer a promising avenue to treat venous leg ulcers. However, the current results are controversial. To further evaluate the efficacy and safety of PTX, we performed an updated meta-analysis of randomized placebo-controlled trials of PTX in the treatment of venous leg ulcers. We systematically searched multiple electronic databases PubMed, Web of Science, Embase, the Cochrane Library, the Cochrane Central Register of Controlled Trials, China Science and Technology Journal Database, WanFang Data, China National Knowledge Infrastructure, and the Chinese Biomedical Literature Database to identify eligible studies. Randomized clinical trials of pentoxifylline versus placebo treatment in patients with venous leg ulcers were considered for inclusion. The primary outcomes included ulcer healing rate and the incidence of adverse events after treatment. The secondary outcomes were the ulcer significant improvement (the ulcer size shrank by more than 60% after treatment) rate, mean duration of complete wound healing and changes in mean ulcer size. A meta-analysis and qualitative analysis were conducted to estimate endpoints. A total of 13 randomized clinical trials, including 921 individuals, were finally included. Compared with placebo, pentoxifylline significantly improved the ulcer healing rate (RR = 1.59, 95%CI 1.22 to 2.07, P < .001) and significant improvement rate (RR = 2.36, 95%CI 1.31 to 4.24, P = .004) while increased the incidence of gastrointestinal disturbances (RR = 2.29, 95%CI 1.04 to 5.03, P = .04) at the same time. Moreover, pentoxifylline also shortened mean duration of complete wound healing (P = .007) and shrank ulcer size (P = .02). Currently available evidence suggests that pentoxifylline could help venous leg ulcers heal more quickly and effectively. However, the evidence is insufficient to prove the results due to moderate-certainty evidence. Large-scale, well-designed randomized clinical trials are warranted.
