Results of Patch Testing in Pediatric Patients with Plantar Dermatitis and Literature Review.

We aimed to determine the frequency of positive patch test reactions in pediatric patients with plantar dermatitis. Children diagnosed as inflammatory dermatitis of the plantar foot were included. Demographic and clinical characteristics of the patients were recorded, and patch testing was done using the European standard series antigens. Seventeen patients (9 females, 8 males) were included in the study. Ages of the patients ranged from 4-13 years, and the median age was 7.0. Seven patients had one to three positive reactions, seven had all negative reactions, one showed angry back syndrome, and results of two could not be assessed. Dermatitis of the feet is not rare in pediatric patients, and a significant percentage of these patients will be diagnosed as allergic contact dermatitis (ACD) if detailed and appropriate patch testing is done.

In industrialised countries atopic dermatitis (AD) is the most common chronic inflammatory disease among children. Many factors influence this disease in a negative way and contact allergy is one such factor. The aim of the study was to examine the frequency of contact allergy among children with the diagnosis AD in Malmö, Sweden and to evaluate if a significant degree of contact allergies in these children is missed when not patch testing them. Contact allergy was found in 22/82 children (26.8%). The most common contact allergies were to Amerchol L101 (11.0%), potassium dichromate (7.3%), and nickel sulphate (4.9%). A statistically significant difference in contact allergy frequency was demonstrated for those with hand and/or foot eczema compared to those without. In conclusion, children with AD should be tested for contact allergy more often, especially in situation with therapy resistance and severe dermatitis, since this may indicate allergic contact dermatitis.

BACKGROUND/OBJECTIVES:
Footwear dermatitis is a form of contact dermatitis resulting from exposure to shoes. There have been only small studies regarding foot contact dermatitis in children. The present study was undertaken to define the prevalence and epidemiologic and clinical features of shoe dermatitis in children.
METHODS:
A retrospective study was undertaken of all children referred for patch testing between 1996 and 2015. Children with dermatitis limited to the feet were selected.
RESULTS:
We collected data from 389 children younger than 16 years, 52 of whom (13.4%) were referred with dermatitis exclusively on the feet. Diagnosis after patch testing was allergic contact dermatitis in 23 children (44.2%), atopic eczema in 12 (23.1%), juvenile plantar dermatosis in 8 (15.4%), dyshidrotic eczema in 6 (11.5%), irritant contact dermatitis in 2 (3.8%), and tinea pedis in 1 (1.9%). The most frequent allergens were potassium dichromate, thimerosal, cobalt chloride, mercapto mix, colophonium, mercury, and nickel(II) sulfate.
CONCLUSION:
Allergic contact dermatitis caused by footwear is a common cause of foot dermatitis in children. Children with foot dermatitis should be referred for patch testing when an allergic origin is suspected.
