Evaluation of balance and fall risk in patients with plantar fasciitis syndrome

Objective: We aimed to investigation of plantar fasciitis syndrome on balance and risk of falls.
Material and Method: Fifty patients with clinical diagnosed plantar fasciitis were enrolled to the study. Patients were evaluated with visual analog scale (VAS) for pain. Balance and fall risk were measured with the Biodex Balance system. Postural stability and fall risk were measured total score (TS), antero-posterior (AP) and medio-lateral (ML) as statically and dynamically at 2. and 8. degrees. 19 healthy volunteers with no active complaints and no previous plantar fasciitis/calcaneal spur were included in the study as a control group.
Results: The mean VAS value in the patient group was 6.65 ± 1.84. There was no statistical difference between the mean age and body mass index of patients and control groups. A statistically significant difference was found between the group in TS2 (p=0,005), TS8 (p=0,009), AP2 (p=0,006), AP8 (p=0,018), DR2 (p=0,01) ve DR8 (p=0,002) in favor of the control group. There was no statistical difference between the groups in the static and dynamic medio-lateral balance evaluations.
Conclusion: These findings demonstrate that postural balance, especially in antero-posterior plane, is impaired in patients with plantar fasciitis syndrome and increased risk of falls.

Greater Postural Sway and Tremor during Balance Tasks in Patients with Plantar Fasciitis Compared to Age-Matched Controls

Plantar fasciitis (PF) is a common condition found in men and women and can reoccur throughout life. PF is commonly diagnosed by prolonged foot pain lasting more than 3 months and a plantar fascia over 0.4 mm thick, as measured using ultrasound imaging. This study examined the ability to balance and the occurrence of muscle tremor during different balance tasks in patients with PF compared to their control counterparts. Fifty subjects (25 patients with PF and 25 control subjects) participated in this study. Subjective pain (measured with a visual analog scale (VAS)), pressure pain threshold (PPT), and postural sway and tremor during eight different balance tasks were measured. Postural sway was measured by a balance platform, while tremor was measured as the mechanical movement of the platform in the 8 Hz frequency range. Thickness of plantar fascia, subjective pain, and PPT were significantly greater in the PF group compared to the controls (p < 0.001). Postural sway and 8 Hz tremor were significantly greater in the PF group compared to the control group for all eight balance tasks (p < 0.01). These results indicate that the lack of plantar fascia elasticity is probably the cause of the reduced balance and increased muscle tremor.

Lower extremity injuries are the most common cases of athlete’s injury. Sports athletes often experience perturbations that disturb their balance. Poor balance is associated with an increased risk of injury in athletes. Plantar fasciitis affects the biomechanics of the foot and impairs pressure distribution and balance during walking and running. To compare the dynamic balance between athletes with plantar fasciitis and without plantar fasciitis. This analytic observational with a cross sectional retrospective study design was performed on 101 athletes, aged 16-32 years. There were 49 male athletes and 52 female athletes have undergone dynamic balance checked with the star excursion balance test (SEBT) and plantar fasciitis examinations based on ultrasonography (USG) examination. There was no difference in the normalized SEBT value between the athlete’s group with plantar fasciitis and the athletes without plantar fasciitis, with p value for the anterior direction = 0.317, anterolateral = 0.215, lateral = 0.062, posterolateral = 0.180, posterior = 0.246, posteromedial = 0.354, medial = 0.409, and anteromedial = 0.245. In summary, there is no difference in the dynamic balance between athletes with plantar fasciitis and without plantar fasciitis. Further research is needed to determine the compensation mechanism in athletes with plantar fasciitis and specific exercises for different sport.

Purpose
To assess if plantar fasciitis has an impact on postural control and walking pattern from gait analysis across different experimental conditions.

Methods
Thirty participants (n =15 with plantar fasciitis) performed 5 different balance tasks on a force platform, and the center of pressure (COP) was computed for postural control analysis. Participants were also asked to walk at 3 different speeds on a gait analysis system to compute the spatial-temporal parameters. Clinical foot measurements (pain, mobility) were also collected through all participants.

Results
Clinical foot measurements showed no significant difference between the two groups; except for pain palpation in plantar fasciitis group. Significant differences were observed between the two groups for COP area displacement sway (p < 0.01; d = 0.08) and velocity (p = 0.022; d = 0.04), where the fasciitis group reported poorer postural control than control mainly during more challenging balance tasks (semi-tandem, unipodal). Plantar fasciitis group reported a decrease of gait velocity (p < 0.01; d = 0.12), step length (p < 0.01; d = 0.16) and step width (p < 0.01; d = 0.18) when compared to the healthy group across walking speed tests.

Conclusions
Individuals with plantar fasciitis report poor postural control and changes in walking pattern across three speeds performance.

Comparison of static balance and gait between subjects with plantar fasciitis and age-matched controls

Background: Plantar fasciitis (PF) is a common condition amongst athletes as well as in non-sporting population. It is characterised by a sharp pain under the calcaneus during walking. The impact of pain due to PF on gait and static balance is examined in this observational study.

Objective: To compare gait and balance between individuals with PF and age-matched controls without PF.

Method: A cross-sectional observational study was executed in an Outpatient Rehabilitation Centre. Twenty-nine participants were included, 14 with PF, and 15 age-matched healthy asymptomatic individuals. Main outcome measures were foot pain, foot function index (FFI), static balance measured with modified Romberg test, static balance measured on the TYMO® system, and gait with the G-Walk System.

Results: Foot pain and FFI were adversely related to balance and gait parameters in subjects with PF. Static balance with eyes open and eyes closed on firm and soft surface measured on the TYMO® balance platform as well as gait parameters measured with the G-Walk system, were significantly lower in subjects with PF compared to age-matched healthy controls.

Conclusion: PF negatively affects parameters of static balance measured with TYMO® system and gait parameters measured with the G-Walk System. However, the Romberg balance test did not detect differences between subjects with PF and age-matched healthy controls.
