    Hi All, just had my first case of plantar plate tear dx via ultrasound. The tear is 4mm full thickness.

    Will begin trial of offloading and possibly taping the 2nd digit, but am wanting advise on how far to try and take conservative therapy before referring to a surgeon for opinion?
    Any help would be fantastic, Ta

    Try taping and putting in rigid post op shoe or similar with suitable offloading of the 2nd MTPJ.

    Move to orthotic and stiff sole jogger etc after a few weeks.

    If ongoing pain and instability after 2-3 months, then delayed primary repair is indicated. Exhaust conservative care first.

    Hi Liz:
    I'd be interested in what the forefoot looks like.
    Is there subluxation? Abnormally long 2nd met?
    What's the first ray look like?

    I have to say, I have just never seen a non-sports related plantar plate injury on anyone who does not have a contracted 2nd MTPJ.

    Anyone else?
