Predictors and persistence of foot problems in women aged 70 years and over: A prospective study.

OBJECTIVE: To examine the prevalence and correlates of foot problems in older women over a 6-year period.

STUDY DESIGN: Women aged 70-75 years who participated in the Australian Longitudinal Study on Women's Health completed a postal questionnaire incorporating questions relating to demographics, major medical conditions and health status in 1999 (n=8059) and 2005 (n=4745).

MAIN OUTCOME MEASURES: Self-reported foot problems at baseline and at 6 years follow-up, major medical conditions, body mass index (BMI).

RESULTS: At baseline, 26% of the sample reported foot problems. At follow-up, 37% remained free of foot problems, 36% had developed a new foot problem, 13% experienced resolution of their foot problems and 14% experienced persistent foot problems. Increase in BMI was significantly associated with the development of new foot problems and the persistence of existing foot problems.

CONCLUSIONS: Foot problems are common in older women and are associated with increased BMI. Maintaining a healthy bodyweight may therefore play a role in the prevention of foot disorders in older women.

I can agree with the findings of the study above, one of my easiest patients is a 99 (very nearly 100) year old woman, who comes to me for her nails cutting because she cannot see quite clearly enough. That's it! no corns or callus, or anything! She is bordering on underweight and does not suffer from rheumatoid arthritis. (Big help!)

Yes, I have the same problem here in the State of Maine. Lots of elderly, no family to help them and the system denies coverage because they have no qualifying factors of diabetes or something similar.
I guess we need more studies / research to help prove to the beaurocrats that people like your patient may be at more risk of injurying themselves or making a simple problem into a more expensive one.

Onset and Persistence of Disabling Foot Pain in Community-Dwelling Older Adults Over a 3-Year Period: A Prospective Cohort Study.

BACKGROUND: Foot pain and related disability in older adults are common yet understudied problems. This study aimed to determine the onset and persistence of disabling foot pain in community-dwelling older adults over a 3-year period.

METHODS: A 3-year follow-up postal survey was conducted in a population sample of older adults aged 50 years and older, recruited previously as part of the North Staffordshire Osteoarthritis Project. Disabling foot pain was defined as the report of problems on at least 1 of the 10 function items of the Manchester Foot Pain and Disability Index occurring on most/every day(s).

RESULTS: Of persons without disabling foot pain at baseline, 8.1% had developed it at 3 years. Onset was greater with increasing age (50-59 years, 6.7%; 60-69 years, 9.1%; and ≥70 years, 9.5%; p = .037), in females (2.5% difference; 95% confidence interval 0.3%-4.8%), and in those with nondisabling foot pain at baseline than those without foot pain (14.2% difference; 95% confidence interval: 10.0%-19.1%). Persistence of disabling foot pain at 3 years was 71.7%, more common in females (9.3% difference; 95% confidence interval: 0.8%-18.0%) but not associated with age.

CONCLUSIONS: Accelerated onset with increasing age and frequent persistence suggests considerable public health impact of disabling foot pain as the population ages. Prevention of disabling foot pain in later life should be prioritized and predisposing factors identified as potential intervention targets.

Background
Foot pain has been shown to be prevalent across all age groups. The presence of foot pain may reduce mobility and impact on the ability to undertake activities of daily living. The aim of this study was to determine factors that are predictive of foot pain in a community based sample of the general population.

Methods
This study analysed data from the North West Adelaide Health Study, a cohort study located in the northwestern suburbs of Adelaide, South Australia. Data were obtained between 2004?2006 and 2008?2010, using a self-completed questionnaire, computer assisted telephone interviewing, and a clinical assessment. The sensitivity, specificity and positive predictive values of variables were determined and generalised linear models ascertained the variables associated with the highest relative risk of self-reporting foot pain in 2008?2010 based on the data obtained in 2004?2006.

Results
The prevalence of foot pain in 2004?2006 was 14.9 % (95 % CI 13.6?16.4) and in 2008?2010, 29.9 % (95 % CI 27.5?32.5). Variables with the highest sensitivity were: female sex, ever having back pain, self-reported arthritis, body mass index (BMI) classified as obese and having foot pain in 2004?2006, while most variables demonstrated high specificity. Those with the highest risk of reporting foot pain in 2008?2010 were those with depressive symptoms, self-reported arthritis, high BMI, self-reported upper limb pain and foot pain (in general or in specific regions of the foot) in 2004?2006.

Conclusion
Foot pain is common in the general population and those with the greatest risk of foot pain potentially represent a high level of chronicity and potential burden on the health system. Addressing the factors that predict foot pain, as well as the provision of targeted messages to highlight the importance of managing foot pain, may help reduce the impact on the population.

BACKGROUND:
The association of quantitative gait characteristics, such as gait variability with musculoskeletal pain is poorly understood. We aimed to examine whether gait speed and gait variability are associated with musculoskeletal pain assessed by the severity and the number of sites in community-dwelling elderly individuals.
METHODS:
A total of 176 elderly individuals participated in this study. The wireless motion-recording sensor units were attached to the lower trunk and heel during gait, and an autocorrelation coefficient was calculated in three directions as parameters of gait variability of trunk movement. Musculoskeletal pain was assessed in two aspects: severity and the number of sites.
RESULTS:
Moderate/severe pain intensity was significantly associated with slow gait speed and low AC in a mediolateral direction (P=0.024 and 0.026, respectively). Participants with musculoskeletal pain in multiple sites had significantly lower autocorrelation coefficient in mediolateral direction than did those without pain (P=0.003).
CONCLUSIONS:
Presence of moderate/severe pain intensity in at least one site or any-intensity pain in multiple sites is associated with slower gait speed and higher gait variability of trunk movement in well-functioning elderly individuals living in the community. Additional studies are necessary to elucidate the causal relationships between musculoskeletal pain and gait.
