I heard a lecture last year where the speaker runs a vitamin D, pth, serum calcium, and 24 hour urine calcium before all fusions. I've read that there is a signficant percentage of low vitamin D levels in patients who sustain stress fractures of the foot. I work in a military environment and anytime I see a stress fracture I run a vitamin D level, usually it's low. I've also found low vitamin D in a patient with chronic shin splints that have been unresponsive to conservative therapy, and in another patient with multiple metatarsal fractures. I've got another patient who smokes and wants a bunionectomy, her vitamin D level was low in the past, and I won't do the case until her vitamin D level is normalized.
