Background: It is well known that interleukin-18 (IL-18) plays a key role in the inflammatory process. However, there are limited data on the role IL-18 plays with diabetic foot ulcers, an acute and complex inflammatory situation. Therefore, we aimed to evaluate serum IL-18 levels of diabetic patients with foot ulcers.

Methods: Twenty diabetic patients with acute foot ulcers, 21 diabetic patients without a history of foot ulcers, and 21 healthy volunteers were enrolled in our study. Circulating levels of IL-18, and other biochemical markers are parameters of inflammation and were measured in all three groups.

Results: Diabetic patients both with and without foot ulcers had high IL-18 concentrations (P < 0.001 and P = 0.020, respectively) when compared with the nondiabetic volunteers. Those with foot ulcers had higher levels of IL-18 level (P < 0.001), high-sensitivity C-reactive protein (hsCRP) (P = 0.001), and erythrocyte sedimentation rate (ESR) (P < 0.001) than those without foot ulcers.

Conclusions: We found that serum IL-18 concentrations were elevated in diabetic patients with acute diabetic foot ulcers. However, these findings do not indicate whether the IL-18 elevation is a cause or a result of the diabetic foot ulceration. Further studies are needed to show the role of IL-18 in the course of these ulcers.

To describe the microbiological profile of diabetic foot infections and what is the
most common microorganism and to determine if the IL 6 can be used as a
predictor of the diabetic foot infection. The study included 80 patients with diabetic
history attended to the outpatient clinic of Diabetic foot, Internal specialized
hospital, Mansoura University. 60 patients were the study group and 20 were the
control group over a period from February 2015 to January 2016. Samples were
cultured and identified microbiologically. The samples were measured fo the IL6
by ELISA. The antibiotic sensitivity test was done. Data for clinical presentation,
bacteriology and management were collected. Statistical analysis was done. 80
patients were included. 60 patients were with infected foot lesions, 7 with
uninfected lesions, 7 have diabetes only with lesion and 6 were normal person.
Gran negative bacteria were more common than Gram negative bacteria. Klebsiella
species were the commonest. Meronem was the most effective antibiotic for the
Gram negative and Gram positive bacteria. There was a significant relation
between the measured IL6 and the study group than the control group but not with
the different microorganism pattern. Gram negative bacteria were the most common isolated bacteria from the diabetic foot infection.
IL6 can be used as a predictor of the diabetic foot infection.

Objective: Diabetic foot ulcers that are one of the factors determined the duration and quality of life
of diabetic patients are one of the most frequent causes of hospitalization and surgery. In this study,
it was aimed to investigate the levels of interleukin (IL) 12 P70, 17, 18, HbA1c, and some parameters
in subjects with type 2 diabetic mellitus, diabetic foot syndromes.
Methods: The study was performed on three groups. They were patients with diabetic foot, patients
with diabetes mellitus type 2 and healthy individuals (control). IL-12 P70, IL-17, and IL-18, HbA1c
levels and other biochemical tests were compared for the three groups.
Results: We found higher IL-17, IL-18, WBC, glucose and HbA1c in the diabetic and diabetic foot
groups than control group. IL-12 P70 levels in diabetic foot group were significantly higher compared
to control group. The performances in diagnosing of IL-12 P70, 17 and 18 as a biomarker for diabetic
foot were statistically significant. IL-12 P70, 17 and 18 levels in diabetic patients were not statically
significant compared with diabetic foot patients.
Conclusion: In this study, it is thought that IL-12 P70, IL-17 and IL-18 for diabetic foot can be used as
biomarkers. Further studies are needed to show the role of IL-12 P70, IL-17 and IL-18 levels in
chronic inflammatory diseases as a biomarker.

Sustained infection and chronic inflammation are the most common features and complex mechanisms of diabetic foot disease. In this study, we examined the expression and functional roles of human endogenous α defensins in diabetic foot ulcer. The expression levels of human α defensins HNP1, HNP3, and HNP4 were significantly higher in the wound center than the edge of diabetic foot ulcers. And the inflammatory cytokine interleukin IL‐8 (IL‐8) was also highly expressed in wound exudates. In human foreskin fibroblasts, these human α defensins were found only slightly to affect IL‐8 expression directly. hemoglobin A1C (HbA1c) is the main clinical indicator of diabetic foot disease. Advanced glycation end products of bovine serum albumin (AGE‐BSA), as HbA1c analogue, was found to promote IL‐8 expression. Human α defensins, in the presence of AGE‐BSA, further significantly promoted IL‐8 expression. These findings showed that human α defensins aggravated the inflammatory response in diabetic foot ulcers patients, providing new insights in to the poor healing of diabetic foot ulcers.

Chronic wounds exhibit persistent inflammation with markedly delayed healing. The significant burden of chronic wounds, which are often resistant to standard therapy, prompts further research on novel therapies. Since the interleukin-17 family has been implicated as a group of proinflammatory cytokines in immune-mediated diseases in the gut and connective tissue, as well as inflammatory skin conditions, we consider here if it may contribute to the pathogenesis of chronic wounds. In this review, we discuss the interleukin-17 family's signaling pathways and role in tissue repair. A PubMed review of the English literature on interleukin-17, wound healing, chronic wounds, and inflammatory skin conditions was conducted. Interleukin-17 family signaling is reviewed in the context of tissue repair, and preclinical and clinical studies examining its role in the skin and other organ systems are critically reviewed. The published work supports a pathologic role for interleukin-17 family members in chronic wounds, though this needs to be more conclusively proven. Clinical studies using monoclonal interleukin-17 antibodies to improve healing of chronic skin wounds have not yet been performed, and only a few studies have examined interleukin-17 family expression in chronic skin wounds. Furthermore, different interleukin-17 family members could be playing selective roles in the repair process. These studies suggest a therapeutic role for targeting interleukin-17A to promote wound healing; therefore, interleukin-17A may be a target worthy of pursuing in the near future.

Elevated Serum Levels of Lp-PLA2 and IL-18 are Associated with Progression of Diabetic Foot Ulcers

Background: Diabetic foot (DF) is a common complication of diabetes with insidious onset, making it difficult for patients to receive timely diagnosis and treatment. This study aimed to evaluate the change in lipoprotein-associated phospholipase A2 (Lp-PLA2) and interleukin-18 (IL-18) in the serum of type 2 diabetes mellitus (T2DM) pa-tients with and without DF, thereby assessing the association between progression of DF and levels of Lp-PLA2 together with IL-18.

Methods: In this study, 50 patients with diabetes without foot ulcers (group I, T2DM group), 135 patients with diabetes with foot ulcers (group II, DF group), and 30 matched healthy controls (group III) were enrolled. Fasting venous blood was collected for detection of inflammatory markers including Lp-PLA2, IL-18, interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), plasminogen activator inhibitor 1 (PAI-1), fibrinogen (FIB), C-reactive protein (CRP), and WBC and neutrophil percentage (Neu%). Baseline indicators such as glycated hemoglobin (HbA1C), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) were tested simultaneously.

Results: The serum levels of Lp-PLA2 and IL-18 increased significantly with T2DM progression and were positively associated with severity of T2DM, with the highest concentrations identified in patients with Wagner grade 4 ulcers (p < 0.05). Univariate logistic regression analysis showed that age, diabetes course, Lp-PLA2, IL-18, FIB, CRP, and WBC and Neu% were risk factors for DF. Multivariate logistic regression analysis revealed that Lp-PLA2, IL-18, FIB, and CRP were independent risk factors for DF.

Conclusions: Increased serum levels of Lp-PLA2 and IL-18 were positively associated with the progression of DF disease. Detection of Lp-PLA2 and IL-18 can assist clinicians' assessment of the severity of DF. Dynamic detection can also help understand disease progression and treatment efficacy.
