BACKGROUND:
Cutaneous manifestations of diabetes mellitus generally appear subsequent to the development of the disease, but they may be the first presenting signs and in some cases they may precede the primary disease manifestation by many years.

AIMS:
The aim of our study was to study the spectrum of dermatoses in diabetics, to know the frequency of dermatoses specific to diabetes mellitus (DM), and to establish the mucocutaneous markers of DM.

MATERIAL AND METHODS:
The study was conducted at a diabetic clinic and our department between September 2008 and June 2010. Two hundred and twenty-four diabetic patients were included in the study group and those with gestational diabetes were excluded. Healthy age- and sex-matched individuals were taken as controls.

RESULTS:
The male to female ratio was 1 : 1.21. Type 2 DM was seen in 89.7% and type 1 DM in 10.3% of the patients. Dermatoses were seen in 88.3% of the diabetics compared to 36% in non-diabetic controls (P<0.05). Cutaneous infections were the most common dermatoses followed by acanthosis nigricans and xerosis in diabetics. Type 2 DM was found to have an increased risk of complications than type 1 DM. Complications of diabetes were seen in 43.7% of the diabetic cases. Diabetic dermopathy, loss of hair over the legs, diabetic foot ulcer, and so on, were found to be the cutaneous markers of DM in our group of cases.

CONCLUSION:
Dermatoses were more common in diabetics than non-diabetics. Cutaneous infections formed the largest group of dermatoses in DM

Introduction: Diabetes mellitus is commonly responsible for skin changes including discrete to mild xerosis.

Areas covered: This review focuses on some selected relevant bioinstrumental methods assessing diabetes xerosis. Peer-reviewed articles on objective non-invasive methods were scrutinized. The reviewed methods address i) the xerosis severity grading scale, ii) corneodynamics referring to the desquamation rate, iii) electrometric assessment of skin hydration including skin capacitance mapping and iv) implication of the imperceptible perspiration. The subjective clinical assessment often fails to disclose diabetic xerosis with confidence and precision. By contrast, a multipronged biometrological approach identifies a cluster of diabetic patients who experience alterations in the structural and functional maturation of the stratum corneum.

Expert opinion: A multipronged biometrological approach helps identifying the changes in the stratum corneum of diabetic xerosis. There is a continuum between the ‘dry skin' feeling, xerosis and ichthyosiform presentations, particularly on the shins and feet of diabetic patients.

Backgrounds
Diabetes mellitus (DM) is a common endocrine disorder and an increasing epidemic worldwide. Proportional diabetic patients eventually develop cutaneous diseases.

Objectives
This study determined the statistical association of cutaneous manifestations and DM as well as the DM-associated cutaneous manifestations in elderly male residents.

Methods
A cross-sectional study was conducted in a Veterans Home in Taiwan. The cutaneous manifestations and major systemic diseases of the residents were recorded separately. Univariate logistic regression and multivariate analysis after adjustment for age, body mass index and significant major systemic diseases provided odds ratios and P values for the statistical association.

Results
A total of 313 male residents (age ≥65 years) were recruited, including 70 (22.4%) with DM. Their most common cutaneous manifestations included fungal infection (77%) and brown spots on the legs (38.3%). Chronic ulcers adjusted odds ratios (AOR 4.90, 95%CI: 1.82–13.19; P = 0.002), brown spots on the legs (AOR 6.82, 95%CI: 3.60–12.89; P < 0.001) and pruritus (AOR 12.86, 95%CI: 4.40–37.59; P < 0.001) were significantly associated with DM. The diabetic residents were inclined to have chronic ulcers, brown spots on the legs and pruritus at a 7.46-fold higher risk (AOR 7.46, 95%CI: 3.86–14.43; P < 0.001). The diabetic residents exhibited marginally higher risks of bacterial infection, scabies, or skin tags.

Conclusion
The DM-associated cutaneous manifestations were chronic ulcers, brown spots on the legs, and pruritus. By observing clues of diabetic cutaneous features, a more complete condition of diabetic patients can be appreciated. The information is essential for providing appropriate treatment and key nursing points regarding the diabetes-associated skin diseases.

Introduction: The skin is the largest and the most visible organ of the body. It is a well known fact that it is referred to as window or mirror to the internal health of the body. Abnormalities of insulin and elevated blood glucose levels lead to metabolic, vascular, neurological and immunological abnormalities. Affected organs include the cardiovascular, renal, nervous system, eyes and the skin. The skin manifestations can be the first presenting sign of diabetes but more often appear in known diabetic patients during the course of the disease. Aim: To study the clinical profile of cutaneous lesions in diabetic patients and to compare the pattern of these dermatoses with HbA1C levels (glycosylated haemoglobin). Materials and Methods: Three hundred consecutive diabetic patients with cutaneous manifestations who attended dermatology OPD at Rajah Muthiah Medical College, Chidambaram from November 2013 to October 2015, constituted the study group. Hundred age and sex matched non diabetic patients who attended our OPD, constituted the control group. They were subjected to relevant investigations. Fasting blood sugar, post prandial blood sugar, glycosylated haemoglobin levels and renal function test were done for all patients. Potassium hydroxide mount and gram stain were done for relevant cases. Chi-square test was used for statistical analysis using software SPSS version 21. Results: The most common dermatological manifestation among diabetics was cutaneous infections, seen in 126 (42%) patients. Among the infections, fungal infections predominated in 89 (29.7%) patients, followed by bacterial infections in 31(10.3%) patients. There was a statistically significant correlation between infections and uncontrolled diabetes (HbA1C >7). Conclusion: Skin is involved quite often in diabetics, some of which can be a consequence or can be a clue for the diagnosis of underlying diabetes. Diabetic control is very important in reducing the morbidity and mortality of the diabetic patients.

Influence of Peripheral Neuropathy and Microangiopathy on Skin Hydration in the Feet of Patients With Diabetes Mellitus.

INTRODUCTION:
Maintaining adequate skin hydration is crucial in the feet of patients with diabetes because xerotic skin may crack and develop fissures, thereby increasing vulnerability to ulceration and infection. The nervous system is considered the powerhouse for maintaining adequate skin hydration; however, no clinical study has assessed the effect of the nervous system on skin hydration. In addition, it is hypothesized that microcirculation may play an important role in maintaining adequate hydration in patients with diabetes.

OBJECTIVE:
This study aims to evaluate the influence of peripheral nerve function and microvascularity on skin hydration in the feet of patients with diabetes mellitus and compare the effects of these 2 functions on skin hydration.

MATERIALS AND METHODS:
This study included 266 patients with diabetic foot disease. Skin hydration was evaluated using corneometry and microvascularity by measuring the transcutaneous oximetry (TcpO₂) of the foot. The Semmes-Weinstein 5.07/10-g monofilament test, electromyography, and nerve conduction velocity test were conducted to evaluate peripheral neuropathy. Patient data were divided into 3 subgroups according to test values, and statistical comparisons were performed using the linear-by-linear association trend and Pearson's chi-square tests.

RESULTS:
There was a significant correlation between skin hydration and TcpO₂. However, there was no significant correlation between skin hydration and peripheral nerve function (P = .338).

CONCLUSIONS:
The results of this study demonstrated that skin hydration in the feet of patients with diabetes mellitus mainly is influenced by microcirculation rather than peripheral nerve function.

number of drugs available against it, diabetes still represents a severe risk for acute and chronic cardiovascular, eye,

A number of skin abnormalities are also well known to negatively affect the quality of life of people with diabetes.
Nevertheless they are not fully described in specialized textbooks and are therefore mostly ignored by family doctors
and diabetologists.
Our paper tries to fill this gap by drawing clinicians’ attention onto skin changes in people with diabetes and to be
aware of those changes in order to identify the disease early enough to prevent more dangerous complications.

The objective of the study is to explore the status quo of foot pruritus and related factors in elderly diabetics and provide a reference for targeted preventive measures. The study involved a survey using a self-designed foot pruritus assessment scale to understand the status quo of foot pruritus among 411 cases of elderly diabetics from 5 communities in Shanghai. The morbidity rate of foot pruritus in elderly diabetics in the community was 20.1%. Good self-management behaviour was the protective factor, while diabetic peripheral neuropathy, hyperlipidemia, and dry skin were risk factors (all P < 0.05). The incidence of foot pruritus in elderly diabetics was high and influenced by several factors. We recommend that self-management behaviour of patients be improved. Additionally, screening and interventions to address hyperlipidemia, diabetic peripheral neuropathy, and dry skin should be conducted regularly to prevent diabetic foot ulcers.

Topical agents have been found effective in managing pruritus, xerosis, and other complications associated with diabetes. Skin care, especially foot care, is crucial in diabetes. Emollients and urea-based creams are commonly used for foot care. The review highlights the importance of following a skin care protocol to prevent skin-related complications in diabetes. Topical agents, emollients, and foot care are crucial in managing diabetic skin problems. Clinicians should ensure that patients with diabetes are educated about skin care's importance and provided with appropriate guidance to maintain healthy skin.

Diabetes mellitus (DM) is one of the world's most important health problems, affecting more than half a billion of the world's population today, with an ever-increasing prevalence. Among the most common manifestations of diabetes are skin manifestations, with 30-70% of patients experiencing skin complications during the course of the disease. Conditions such as acanthosis nigricans, diabetic dermopathy, necrobiosis lipoidica, bacterial infections, fungal infections, skin xerosis, and metabolic prurigo are often associated with diabetes and often precede its diagnosis. We conducted a retrospective study on a group of 103 patients hospitalized between January 2018 and December 2022, in a clinic of a county hospital, using as criteria the diagnosis of diabetes mellitus complicated by cutaneous manifestations frequently associated with diabetes. The aim was to observe which are the most common manifestations and whether they correlate with data in the research literature. In the present study, manifestations such as diabetic foot (20% of patients), bacterial (35%) and fungal infections, and cutaneous xerosis (45%) were predominant. Often, the integumentary involvement may precede the diagnosis of the underlying disease. It is therefore very important to recognize, investigate and treat these manifestations as soon as possible.
