Every now and then I will notice that patient’s feet will slide laterally off the side of athletic shoes when in stance. Meaning the lateral upper will be very snug at the point of the 5th metatarsal head, part of the lateral border of the foot will rest off the midsole and the medial side off the upper is loose.

I have noticed this in neutral style shoes, no medial wedging to midsole and with no orthotics. The patients foot type is rectus forefoot (definitely not forefoot valgus) and Fhl. Has anyone else noticed this phenomena? I have been trying to figure out this out, so I can address it. I am beginning to believe that the patient do not have enough functional dorsiflexion motion at their 1st MPT to accommodate for inclination of the 1st metatarsal to the floor. Therefore compensating with a lateral shift to the foot. But, I would like to hear other possibilities.
