I have a patient who has Multiple sclerosis who is beginning to lose a bit of the feeling in her feet and is as a consequence becoming more unstable in her gait.
Does anyone have any exprience of using textured/proprioceptive insoles in such cases and did you feel that they were of benefit?If so what insole did you use?The only one I can come up with is the croc insole which has dimples on it.I have toyed with the idea of putting cous-cous on a bit of leatherboard and using a very thin top-cover,but that seems awfully crude.
Your input on different insoles(and dried comestibles) would be appreciated,

Firstly, Its exteroceptive not proprioceptive.
Secondly, rather than adhere foodstuffs to insoles, look to a company like algeos who provide shoe soling material and try that for dimply / textured materials. -
Thanks for your reply Robert(and correction!).I wasn't seriously thinking of the cous-cous option.I have spoken to someone at Algeos who couldn't think of using any of their insoles for my purpose.I presume you would turn outer sole upside down and stick it in shoe?The study on M.S. website suggested Crocs or another insole supposedly from Algeos but as I say they were non the wiser,

Rome K, Richie DR, Hatton AL. Can orthoses and insoles have an impact on postural stability? Podiatry Today 2010; 23: 45-51.
Contains eight references in the section on the effect of functional foot orthoses on postural stability and three in the section on the effect of textured insoles on balance and stability.
Of those three papers in the section on textured insoles, two 1,2 show a significant change in stability / gait kinematics with textured insoles, in some patient groups. The other study (which Prof Rome co-authored) showed no significant change in postural stability with textured insoles in either static or dynamic tests 3
Another very thorough paper co-authored by Prof Rome (Hatton et al 2009) looked at the effect of textured surfaces on postural stability and lower limb muscle activity and found that the textured insole did not significantly effect either postural sway or lower limb EMG readings in eight dominant lower limb muscles 4. This rather good study is not mentioned in the section on the effects of textured insoles, but does appear elsewhere in the review.

We have looked at textured insoles in MS. In fact, we have just been funded by MS Society UK into the long-term effects of textured shoe insoles on balance, walking ability and function in people with MS.

Hiya,
Textured soles are a good option, however each patient needs to be assessed individually.
My mother has M.S. and resultant neuropathy means even the small nodules in the bottom of a foot spa can lead to soft tissue damage, as she is unable to always sense when a pressure area is forming and move her feet accordingly.
My apologies that I have not explained myself very well.
My point is that you want a more uniform texture rather than spaced-out raised areas, to reduce areas of peak pressure. Also that what suits one may not suit another.
