Can anyone advise me, I'm treating a lady in her 30s who is fit and healthy goes on winter sport holidays etc. wears loose fitting soft boots. Has a HD dorsal lateral side of 5th digit nail that comes back after two weeks is 2mm deep. Causes pain good arches no leg length problems. Last treatment I put a blank insole in her boot with a lateral posting , I've been a Chiropodist for nearly twenty years and think I can remove a corn but why does it come back so fast help :boohoo:

 

99 times out of a 100, this type of corn is caused by GRF on a varus, underlying 5th toe. If this is the case, then referral to a pod surgeon who will carry out a de-rotational arthroplasty/arthrodesis will sort the problem out.

Put gel toe cap on it or silicon mould!?

 

I have had a lot of success by performing an ostectomy of the base of the distal phalanx to decrease the offending pressure. The toe may still be in varus but corn resolves and the patient is asymptomatic. If it was my 5th toe, I would choose the ostectomy, if it failed to resolve the painful corn, I could always elect the derot arthroplasty. Arthrodesis contraindicated in the 5th toe.

Thanks everybody for your help . I will try a good gel cap next time and if that's not working see about refer for surgery.

 

Change the lateral GRF with appropriate orthotic prescription or in shoe padding.
