    Dear all,

    Just wondering what the current range of opinions is regarding orthotic management of significant flexible and rigid cavo-varus, which corrects the hindfoot to neutral with a Coleman block test (ie FF valgus/RF varus scenario).

    Is there any preference for Fettig/Denton/Feehrey modifications or similar.

    I seem to never quite get the amount of control I need to correct the hindfoot to neutral when bringing the "ground up" to meet the lateral column...

    A podiatrist by the name of Joshua Burns has just submitted his PhD on the best orthosis for the pes cavus foot type. I hope Josh will jump in and post, but what he presented at the recent Australasian Podiatry Conf suggested that the pt's faired best with a device that had:

    * Modified Root device
    * 20% medial arch fill (ie. maximum expansion)
    * Lateral heel post (with no taper/grind - almost a lateral heel flare). No posting on the medial side of heel.
    * 3mm Poron full length covering + Cashmere
    * 12mm heel cup height
    * 6mm lat heel expansion (mainly due to the thickness of the poron)
    * F/ft width - lateral border of foot to bisection of 1st MPJ
    * Length at distal edge was proximal to MPJ's

    I am not sure in which publication/s the research is to be submitted, but will post as soon as the details come to hand.
