Hi,

I've been searching the net for copies of;

The Foot Health Status Questionnaire (FHSQ)
The Bristol Foot Score
The Podiatry Health Questionnaire (PHQ)

but have so far been unsuccessful. Can anyone advise me where I can obtain these or are they only available under license??

I have also got a query about the Foot Health Status Questionnaire, what I am wondering is if anyone knows of how to get hold of the FHSQ software?

Any help would be greatly appreciated :)
