Cracking resource here with some lovely images of surgical procedures of the foot and ankle. Completely free but you will need to register after viewing 5 or so operative steps.

Worth doing even if your practice or personal interests don't have a huge surgical slant as it will be updated with more procedures and also expand to include more rehab and biomechanics in due course.
