    Hi all,

    I haven't posted before. I am looking for some advice on footwear for an elderly patient who has severe diabetic neuropathy and poor vascular condition. She has had BL hallux amputation and has presented with an ulcer under callous of one suture line. Currently wearing inappropriate floppy slippers

    In my practice, I see several such patients. I have an excellent pedorthist nearby, who specializes in difficult cases- severe PVD, neuropathic feet- he fabricates custom, accomodative orthoses and includes less-dense filler material where amputations exist. Also, extra-depth shoes and usually some type of rocker-bottom sole are included.

    We are also doing tendo-Achilles lengthening procedures for some forefoot ulcer patients.

    The Type of footwear or indeed brand is not the big all important thing here...the correct fit is and a flip flop or a slip on of any type will not be able to give that to you and your client, they might be able to go into an off the shelf depth product otherwise they might need custom made your local Pedorthist will make that call once they have seen the foot but a rocker sole is going to have to be part of the shoe selection either way

    l suggest you will need a lace up best if not a velcro will have to do, we must consider it all comes back to the clients dexterity and/or who dresses them?

    l agree the client with diabetic neuropathy should be fitted with a total contact orthosis(TCO). This is very important even more so when you consider "...an ulcer under callous of one suture line.." once you have cleaned it up TCO will help greatly.

    There is Ben & Jody Playford "Footwear Plus" in SA.

    Give them ago and see how you go.
