Can anyone recommend one?

I'd like a general text and probably one focussing on biomechanics too.

At the moment, I'm only really aware of Neale's Disorders of the Foot and Clinical biomechanics of the lower extremities by Valmassy.

IMHO, there is not one. Generally, the more comprehensive (greater coverage) a text becomes, the less detail is covered on each topic. ie the comprehensive texts don't realy go into enough depth on each topic. The more specialised texts that go into topics in more depth are not as comprehensive in their coverage.
