First metatarsophalangeal joint- MRI findings in asymptomatic volunteers

Objectives
To evaluate the spectrum and frequency of MR findings of the first metatarsophalangeal joint (MTPJ) in asymptomatic volunteers.
Methods
MR imaging of 30 asymptomatic forefeet was performed with a dedicated extremity 1.5-Tesla system. Participants were between 20 and 49 years of age (mean ± SD: 35.5 ± 8.4 years). Two radiologists assessed cartilage, bone, capsuloligamentous structures, and tendons of first MTPJs on MR images.
Results
Cartilage defects were observed in 27 % (n = 8) of first MTPJs, most frequently located at the base of the proximal phalanx (23 %, n = 7), whereas cartilage defects of the metatarsal head (13 %, n = 4) and the metatarsosesamoid compartment were rare (0 %–3 %, n = 0-1). Bone marrow oedema-like signal changes were present in 37 % (n = 11) and subchondral cysts in 20 % (n = 6) of first MTPJs. Hyperintense areas on intermediate-weighted sequences (range: 30–43 %, n = 9–13) and on fluid-sensitive sequences with fat suppression (range: 33–60 %, n = 10–18) within the medial and lateral collateral ligament complex were common. Plantar recesses (77 %, n = 23) and distal dorsal recesses (87 %, n = 26) were frequently observed.
Conclusions
Cartilage defects, bone marrow oedema-like signal changes, subchondral cysts, plantar recesses, and distal dorsal recesses were common findings on MRI of first MTPJs in asymptomatic volunteers. The collateral ligaments were often heterogeneous in structure and showed increased signal intensity.

Hallux Valgus Evaluation on MRI: Can Measurements Validated on Radiographs Be Used?

Hallux valgus (HV) is a common deformity of the great toe affecting >23% of adults in the United States. The severity of the deformity is traditionally analyzed using radiographs to determine measurements such as the HV and intermetatarsal angles. We sought to determine the relationship between the radiographic and magnetic resonance imaging (MRI) measurements because this is not yet known. Two of us analyzed a series of 56 consecutive patients who had had radiographs and MRI performed on the same foot between April 27, 2015 and March 9, 2016 and who satisfied all other inclusion and exclusion criteria (age 18 to 100 years, no history of recent foot trauma, and no metal hardware in the foot). We found excellent interreader reliability (intraclass correlation 0.89 to 0.96) and intermodality agreement (intraclass correlation 0.83 to 0.91). The HV angle measured 15.0° ± 8.8° on the MRI scans and 13.8° ± 8.7° on the radiographs (mean difference −1.15° ± 3.89°), and the intermetatarsal angle was 9.0° ± 3.1° on the MRI scans and 8.8° ± 2.9° on the radiographs (mean difference −0.22° ± 2.10°). The HV measurements were reliable on both radiographs and MRI for the range of values tested. Small intermodality statistically significant differences in HV angle measurements were found; however, these might not be enough to be clinically significant.

Point vs. traditional method evaluation of hallux valgus: interreader reliability and intermethod performance using X-ray and MRI

Background
The two most widely used measurements for diagnosing and assessing the severity of hallux valgus are the hallux valgus angle (HVA) and the intermetatarsal angle (IMA). Traditionally, these have been measured by using the midaxial lines approximating the axis of each bone. A new simpler point method has been recently suggested for measuring these angles by connecting points along the medial corners of each bone. Interreader reliability of these measurements on X-ray and MRI as well as intermethod and intermodality differences have not been assessed.

Methods
A series of 56 consecutive patients between 18 and 100 years old with no history of foot trauma or orthopedic hardware in their feet were included. All had AP and lateral X-rays and MRI performed on the same foot between April 27, 2015 and March 9, 2016. Two readers measured HVA and IMA using both the traditional midaxial and new point methods. ICC correlations were obtained.

Results
The interreader reliability for HVA was similar on point method (0.92) and traditional method (0.94). For the IMA, the ICC was 0.77 on point method versus 0.76 on traditional method. The intermodality agreement (between X-ray and MRI) was higher for HVA (ICC = 0.85, 0.88) as compared to IMA (0.58, 0.74), respectively on both methods. The mean difference between the methods was larger on traditional method = 5.5 for HVA and 2.5° for IMA.

Conclusions
HVA is more reliable than IMA on both methods and modalities and a significant difference exists between the magnitudes of values obtained using the two methods.

Accuracy of magnetic resonance imaging for diagnosing hallux valgus

Objective:
To assess the accuracy of magnetic resonance imaging (MRI) for the diagnosis of hallux valgus using radiography during weight bearing as the gold standard.

Materials and Methods:
This was a retrospective analysis of all patients undergoing MRI of the foot and radiography of the foot during weight bearing at our institution between January and June of 2015. The hallux valgus angle (HVA) was measured on MRI and radiography. The Wilcoxon signed-rank test and simple linear regression were used in order to compare measurements. Patients were divided into two groups according to the HVA determined on radiography: > 15° (hallux valgus) and ≤ 15° (control). Qualitative and quantitative assessments of MRI scans were performed. For quantitative assessment, receiver operating characteristic curves were used in order to determine the HVA cutoff with the highest accuracy.

Results:
A total of 66 MRI scans were included, 22 in the hallux valgus group and 44 in the control group. Wilcoxon signed-rank tests indicated a significant difference between the radiography and MRI measurements. Simple linear regression showed a nonlinear relationship between the measurements and values did not present a strong correlation. In comparison with the radiography measurements, MRI with an HVA cutoff of 16.4° exhibited the highest accuracy (86%). The accuracy of the subjective (qualitative) assessment was inferior to the objective assessment (measurement of the HVA).

Conclusion:
Hallux valgus can be diagnosed by measuring the HVA on MRI, satisfactory accuracy being achieved with an HVA cutoff of 16.4°.

The first metatarsophalangeal joint is a complex of supporting structures that is vital to the biomechanics of the foot, and forced hyperextension of the first metatarsophalangeal joint known as turf toe may lead to progressive orthopedic deformities along with eventual degenerative hallux rigidus.

The first metatarsophalangeal joint (MTPJ) is vital to the biomechanics of the foot and supports a weight up to eight times heavier than the body during athletic activities. The first MTPJ comprises osseous and cartilaginous surfaces along with a complex of supporting structures, including the dorsal extensor tendons, collateral ligaments, and a plantar plate complex. In contradistinction to the lesser MTPJ plantar plates, a single dominant fibrocartilaginous capsular thickening does not exist at the first MTPJ. Instead, the plantar plate complex comprises a fibrocartilaginous pad that invests the hallux sesamoids and is inseparable from the plantar capsule, the intersesamoid ligament, paired metatarsosesamoid and sesamoid phalangeal ligaments (SPLs), and the musculotendinous structures. Acute injury at the first MTPJ is typically secondary to forced hyperextension—turf toe—and can involve multiple structures. During hyperextension, the resulting forces primarily load the distal SPLs, making these structures more susceptible to injury. SPL injuries are best seen in the sagittal plane at MRI. Radiography can also aid in diagnosis of full-thickness SPL tears, demonstrating reduced sesamoid excursion at lateral dorsiflexed (stress) views. Hallux valgus is another common condition, resulting in progressive disabling deformity at the first MTPJ. Without appropriate treatment, first MTPJ injuries may progress to degenerative hallux rigidus. The authors detail the anatomy of the first MTPJ in cadaveric forefeet by using high-resolution 3-T and 11.7-T MRI and anatomic-pathologic correlation. Injuries to the plantar plate complex, collateral ligaments, and extensor mechanism are discussed using clinical case examples.
