Nanoengineered biomaterials have tremendously improved the range of tools utilized for the control of as well as acceleration of healing of diabetic foot ulcers (DFU) over the last few decades. Despite nanoparticles and electrospun nanofibers being used extensively for the treatment of DFU, there has been no review available till now which addresses the utilization of the latest nanodelivery techniques along with modern electrospun nanofiber treatments for DFU. This review thoroughly summarizes the latest mindboggling findings of potential nanomaterials like polymeric and metallic nanoparticles and electrospun nanofibers for DFU treatment. In addition, a succinct insight into the potential of the aforementioned nanomaterials which can be exploited as therapeutic delivery agents for improving the regeneration of damaged dermal and epidermal tissues of DFU is underscored. Ultimately, current challenges and future prospects of nanoparticles and electrospun nanofibers for DFU treatment are highlighted.

Diabetes is one of the most prevalent diseases in the world with high-mortality and complex complications including diabetic foot ulcer (DFU). It has been reported that the difficulties in repairing the wound related to DFU has much relationship with the wound infection, change of inflammatory responses, lack of extracellular matrix (ECM), and the failure of angiogenesis. Following the development of medical materials and pharmaceutical technology, nanofibers has been developed by electrospinning with huge porosity, excellent humidity absorption, a better oxygen exchange rate, and some antibacterial activities. That is to say, as a potential material, nanofibers must be a wonderful candidate for the DFU treatment with so many benefits. Careful selection of polymers from natural resource and synthetic resource can widen the nanofibrous application. Popular methods applied for the nanofibrous fabrication consist of uniaxial electrospinning and coaxial electrospinning. Furthermore, nanofibers loading chemical, biochemical active pharmaceutical ingredient (API) or even stem cells can be wonderful dosage forms for the treatment of DFU. This review summarizes the present techniques applied in the fabrication of nanofibrous dressing (ND) that utilizes a variety of materials and active agents to offer a better health care for the patients suffering from DFU.

Diabetic foot ulcer (DFU) is often associated with the risk of non-traumatic lower extremity amputations in diabetic patients. Wound dressing with the active therapeutic agent(s) would be necessary to effectively manage the chronic condition of DFU. Deferoxamine (DFO) has been reported as a potential therapeutic agent in improving the wound healing process by regulating the expression and increasing the stability of hypoxia-inducible factor-1α (HIF-1α). Concurrently, polymeric nanofiber mats offer great potential for delivering therapeutic agents to wound owing to their high surface area to volume ratio and high porosity. Therefore, this study was aimed to develop a biomaterial-based nanofibrous wound dressing containing DFO as a novel platform to increase angiogenesis at the wound environment for rapid healing. A DFO-loaded bilayer nanofiber mat was formulated using chitosan, sodium alginate, and polyvinyl alcohol using electrospinning method. The effects of three independent parameters on nanofibers morphology were investigated using Box-Behnken statistical design. The fabricated bilayer electrospun fiber matrices had displayed the criteria of an ideal wound dressing with a high swelling degree of 594%, sufficient water vapour transmission rate (427.49 g/m2∙day), high entrapment efficiency (98%), sustained drug release up to 48h, and zone of inhibition against Gram + ve (0.79 ± 0.07 cm2) and Gram –ve (0.95 ± 0.04 cm2) organisms. The bilayer nanofiber mat was also found to be non-cytotoxic and the in vitro scratch test revealed wound healing potential, which implied its promising role as wound dressing tool to provide efficient treatment against DFU.

Diabetic foot ulcer (DFU) healing has long been a major medical challenge. The type of dressing is an essential factor in wound healing, prevention of local infection, and scar formation. Today, smart wound dressings or wound healing patches can precisely control drug delivery to the target tissue and prevent this significant complication. Nanofiber (NF) wound dressings are effective in reducing wound scarring and helping to speed up the healing process for DFU. The electrospun NFs have a suitable surface topography, density, and three-dimensional structure, which can be considered an efficient method to produce a substrate for tissue engineering and wound healing. Chitosan (CS) is one of the most well-known biopolymers in wound healing tissue engineering and drug delivery systems. The unique properties of CS make it suitable for biomedical applications. Based on new studies in the field of hemostatic and antimicrobial effects of CS in controlling bleeding and wound healing and application of NF wound dressings, the purpose of this study is a review relevant works on CS-based NFs to improve the DFU.

Diabetic foot ulcers (DFU) are among the most common complications in diabetic patients and affect 6.8% of people worldwide. Challenges in the management of this disease are decreased blood diffusion, sclerotic tissues, infection, and antibiotic resistance. Hydrogels are now being used as a new treatment option since they can be used for drug delivery and to improve wound healing. This project aims to combine the properties of hydrogels based on chitosan (CHT) and the polymer of β cyclodextrin (PCD) for local delivery of cinnamaldehyde (CN) in diabetic foot ulcers. This work consisted of the development and characterisation of the hydrogel, the evaluation of the CN release kinetics and cell viability (on a MC3T3 pre-osteoblast cell line), and the evaluation of the antimicrobial and antibiofilm activity (S. aureus and P. aeruginosa). The results demonstrated the successful development of a cytocompatible (ISO 10993-5) injectable hydrogel with antibacterial (99.99% bacterial reduction) and antibiofilm activity. Furthermore, a partial active molecule release and an increase in hydrogel elasticity were observed in the presence of CN. This leads us to hypothesise that a reaction between CHT and CN (a Schiff base) can occur and that CN could act as a physical crosslinker, thus improving the viscoelastic properties of the hydrogel and limiting CN release.

Diabetic wounds are not easy to repair mainly attributed to their complicated wound situation which can lead to the injure wound further infection and even death of patients. Chitosan-based hybrid dressing materials have recently attracted considerable attention as excellent potential biomedical materials with excellent hydrophilicity, absorbability, anti-inflammatory, and antibacterial properties, and substantial contribution to tissue repairment and regeneration for treating diabetes wounds and accelerating wound healing. In this chapter, we analyze the preparation of the chitosan-based hybrid dressing materials, as well as the applications of these wound dressing toward diabetes wounds. It is believed that chitosan-based hybrid dressing materials will play an essential role in the field of treating diabetic wounds due to their unique performances, such as excellent biocompatibility for increasing the angiogenesis and collagen deposition, cell proliferation, and promoting the aggregation of growth factors, as well as the antibacterial performance for inhibiting the wound inflammation surround the injured wound. The present chapter reveals an overall idea regarding the introduction to chitosan-based hybrid dressing materials for the treatment of diabetic wounds

Treatment of DFU has gained an attention of biomedical engineering as the traditional methods were having various challenges to overcome which, 3D Scaffolds (Nanofibers) have emerged as potential candidates. Nanofibrous scaffolds are a promising approach as they can mimic extracellular matrix, rejuvenate the skin, and promote the wound healing. This review focuses on the delivery strategies of nanofibers, including their fabrication techniques and different polymers utilized in preparation along with their merits and demerits. Furthermore, the applications of nanofibers and recent patents that got approved have been discussed. Overall, this review attempts to highlight the potential of nanofibers in diabetic foot ulcer treatment and the need for further research and development to optimize their use in clinical settings.
