    This sounds like a very intresting one!
    it is very difficult to diagnos somthing like this without being able to see or speek to the patient directly! as there are so many tendons a ligaments around the lateral side of the foot! it sounds like a strain or a ruptchure! i would not suspect a break as it does not hurt when he walks on his tip toes! i would recoment an x-ray just to rule that out!

    As Simon mentions veryb difficult to make a diagnoses. I would certainly recommend getting an xray to rule out any stress fractures. I have seen a patient in the past with a similar complaint, nothing showed up on the xray. However following a 4week course of ultrasound the pain cleared up. Obviously your case is unique but may be worth bearing the ultrasound in minds as an option of treatment.
