    Hi all,
    I'm a Cert 4 Allied Health Assistant currently working as a physio assistant (podiatry jobs hard to come by in my area!) Anyway... I have a resident who has severe RA in every joint. Has just had surgery on the Rt foot to release soft tissue around joint, pin big toe straight with 2nd toe amputated. They were going to do similar to L foot, but surgeon has now told her that the 2nd toe can't be amputated without skin grafts so he doesn't want to do the surgery.

    She is in a great deal of pain with this 2nd toe as it has stiffened into a solid hammer toe. She has asked me if possible to make any kind of insert for her shoe to pad under the toe and ease that rubbing a little.

    Checked her shoes out yesterday and she has orthoses glued into her shoes, (remembering I have little resources available, just what I can track down online) I was thinking of grabbing a sorbothane insole to put in her shoe, then using one of those gel pads for under the toe, but I think this could create a disturbance in balance with her walking.

    Hoping someone else can come up with some ideas for me? Thanks in advance.

    By the time you add all of that to the shoe there will be no room for her foot;).

    Adding the Gel pad under the MPJs is increasing the height of them, meaning more force will be put on the MPJs as your client starts to lift her heels as part of the gait cycle, you dont want that.

    Find your local Pedorthist and ask for them to modify your clients footwear with a stiff rocker sole, here is the link to certified Pedorthist, also refer the client back to the provider of the Orthosis for possible modifications

    how mobile is this lady and how dexterous are her hands? if she can reach then a simple jel tube will help.
