    hmmmmmmm.
    Well, give a marcaine block and make sure the bandage isn't tight and there will be little to no pain for 20-24 hrs.
    Perhaps someone can explain the results of this study to me.
    Is it trying to say that a post op block has no effect on post operative pain management?????
    Ridiculous.

    The study does not specify whether a full ankle block was carried out, although the volume of L.A. used would suggest that it was; however, the abstract does not describe what a 'single shot ankle block' consists of. If, as suggested, the test applied was the need for oral analgesics within the 24 hours post op. then I can understand the lidocaine group requiring this after 1 hour and the levo-bupivacaine requiring oral analgesia from perhaps 6 hours post-operatively. I concur with Steve that personally, I have not found levo-bupivacaine 2.5% as effective as bupivacaine 0.5%, but that is anecdotal.

    If it is the case that a full ankle block has been carried out and that the L.A. is no more effective than saline for, say, the first 6 hours post-op. then one must question the technique.
