Background
Contribution to literature is critical for progress in the field of orthopaedics. No previous study has yet examined the academic productivity of foot and ankle surgery fellowship faculty.

Purpose
To evaluate the publishing productivity of foot and ankle fellowship faculty.

Methods
Faculty and program characteristics of orthopaedic foot and ankle fellowship programs across the United States and Canada were collected from American Orthopaedic Foot and Ankle Society (AOFAS) and program websites. Faculty publication productivity measures, including publications, number of publications in specific journals, number of citations, and Hirsch index (h-index) were gathered using the Scopus database.

Results
A total of 48 AOFAS foot and ankle surgery fellowship programs were identified with an associated 185 faculty members. The mean number of publications per faculty member was 44.9 (SD = 53.0; range = 0-323), with a mean h-index of 11.9 (SD = 10.6; range = 0-54). A total of 144 (77.8%) academic-affiliated faculty had a significantly greater number of publications (P < .01), total citations (P < .05), and publications in Foot and Ankle International (P < .05), Journal of Bone and Joint Surgery (P < .05), Clinical Orthopaedics and Related Research (P < .05), and Journal of the American Academy of Orthopaedic Surgeons (P < .05) compared to the 41 (22.2%) nonacademic faculty. There were no significant differences between measures of publication productivity between male and female faculty, except for maximum citations in a single article (67.1 vs 142.3; P < .05).

Conclusions
Academic-affiliated foot and ankle fellowship faculty have higher research productivity than nonacademic surgeons. The mean h-index of foot and ankle fellowship faculty was 11.9, which is lower than that reported in sports, joints, and spine fellowship faculty but higher than that reported for hand fellowship faculty.

Research Activity Among Foot and Ankle Surgery Fellows: A Systematic Review

Surgical residents cite a number of reasons to pursue a fellowship training program including improving surgical skills, furthering medical research, pursuing an academic practice, or to generally become an overall better trained surgeon and clinician. The interest in foot and ankle surgery fellowships has increased among graduating residents as have the number of fellowship programs. Since the introduction of these programs, there has been no formal investigation of the scholarly activity among foot and ankle surgery fellows. Using PubMed, a systematic review was conducted from papers published by fellows participating in American College of Foot and Ankle Surgeons or American Podiatric Medical Association approved fellowships during 2013 to 2019. A total of 76 of the 128 identified fellows published research during or within one year of completing their fellowship. Fellows that published at least once prior to fellowship were more likely to publish during fellowship compared to those who had no publication history. Over this 6-year period, fellows contributed to 279 manuscripts where they maintained primary authorship of 34.41% of the publications, across 35 journals, with the most common being the Journal of Foot and Ankle Surgery. Results of this study provide a survey of the scholastic activity among foot and ankle surgery fellows and could be used by applicants and evaluators to stratify applicant aptitude. These results could also serve as a scholarly activity benchmark for current fellows and a method of gauging scholarly involvement for new and current fellowships.

A Bibliometric Evaluation of Worldwide Research of the Podiatry Field from 1965 to 2017

Background: To identify the strengths and weaknesses in a given research area, it is necessary to analyze the published literature. International studies on podiatry research productivity are scarce. This study aimed to analyze scientific productivity in the area of podiatric medicine from 1965 to 2017.

Methods: This was a retrospective, observational, bibliometric study. The MEDLINE database was used to identify research published between 1965 and 2017. Literature searches were performed in 2010 and 2017 through RefWorks, and research production per year, author, document type, country, institution, journal, and language were calculated. Podiatry's contribution to global scientific production was measured by calculating the ratio of podiatry publications to total production, and Price's law was applied to analyze the temporal evolution. Author productivity index, coauthorship, geographic distribution, and the distribution by institution type and journal (Bradford's law) were analyzed.

Results: The MEDLINE search yielded 1,256 publications, representing 4.75 articles per 100,000 publications in global scientific research. The growth rate followed Price's law after linear adjustment. The 2,229 identified authors presented a transience index of 85.73%; 0.38% were highly productive authors. The coauthorship index increased from 1.40 in 1965 to 5.80 in 2017. The most common document type was the journal article, whereas 2.1% were clinical trials. Only one document reported a controlled clinical trial. The United States led scientific production, with 77.15% of the documents; 60.5% of the publications were concentrated in four journals.

Conclusions: Podiatry is still an emerging research field, and literature is concentrated in a small number of journals, categorized into different subjects

Four Decades of Foot and Ankle Research Activity: A Scientometric study of Subspecialty Foot and Ankle Journals

Objectives: To evaluate foot and ankle documents using scientometric methods and provide insight into global research activities.

Methods: This scientometric study was conducted at the Department of Spine Surgery, Ganzhou People's Hospital, China. Documents on foot and ankle from 1980 to 2019 were retrieved from the Scopus database. The number of documents, year of publication, journal, country, institution, author, h-index, and top-cited documents were analyzed.

Results: In total, 11313 documents were retrieved. The annual research output on foot and ankle showed a dramatic increase over the past four decades, especially in the past decade (p = 0.000). Foot & Ankle International published the biggest number of documents (44.48%). The United States contributed more than half (52.17%) of the global production, followed by the United Kingdom (11.39%), and Germany (3.62%). The United States had the highest h-index (109). The Hospital for Special Surgery (1.87%) ranked first in terms of productivity, followed by Union Memorial Hospital (1.37%), and Duke University Medical Center (1.24%). The most productive author was Myerson MS (1.25%), followed by Schon LC (0.77%), and Hyer CF (0.74%).

Conclusion: Research on foot and ankle has thrived rapidly over the past four decades, particularly in the last decade. The United States contributes the most to the quantity and quality of foot and ankle documents.

Research Productivity and Impact in Foot and Ankle Surgery: Insights From Relative Citation Ratio Analysis of Recent Fellowship Graduates

Introduction: The relative citation ratio (RCR), a novel bibliometric tool supported by the National Institute of Health, provides a standardized approach to evaluate research productivity and impact across different fields. This study aims to evaluate RCR of fellowship-trained foot and ankle orthopaedic surgeons to analyze the influence of various surgeon demographics.

Methods: Fellow names listed on the American Orthopaedic Foot and Ankle Society website were extracted from the year 2008 to 2009 to the year 2022 to 2023. Demographic information for each fellow was collected including sex, degree type, and academic title. The iCite database developed by the National Institute of Health was used to obtain total publications, mean RCR, weighted RCR, and change in RCR after fellowship graduation for each fellow. Univariate and multivariate analysis was conducted to predict these four parameters based on sex, degree type, academic position, and career longevity.

Results: Of the 820 fellows, 674 (82%) were male. Most fellows (n = 587, 71%) did not go on to hold academic positions. Multivariate analysis revealed that male sex (β = 2.32, P < 0.001), holding an academic position (β = 6.44, P < 0.001), holding a PhD (β = 22.96, P < 0.001), and a shorter length time since graduation (β = -0.50, P < 0.001) were independent predictors of number of total publications. Holding a DO degree was an independent predictor of decreased mean RCR (β = 0.39, P = 0.039). Finally, multivariate analysis revealed that male sex (β = 4.05, P = 0.003), a career in academics (β = 4.61, P < 0.001), and a shorter time since graduation (β = -0.45, P = 0.001) were associated with a larger weighted RCR.

Discussion: The findings highlight the importance of addressing gender disparities and promoting research opportunities across different programs. Moreover, academic institutions should provide adequate support and mentorship to early-career foot and ankle-trained orthopaedic surgeons to foster sustained research productivity.
