    Hi,

    I am interested to know if any organisations have a formalised tool or system for managing and prioritising existing patients.

    My organisation is looking to introduce a prioritisation tool for existing patients (as a form of demand management so we can triage care for existing patients). This aims to standardise the assessment practices and categorise care according to need.
    Most allied health appointments are episodic (short levels of treatment), so that patients have to be triaged at the start of every episode, and then they are discharge. Podiatry has episode and ongoing levels of treatment. Some podiatry patients are seen over a period of years, and some never get discharged..

    Does anyone know of any tool in podiatry or allied health tool that categorises/ prioritises on-going patients?.
