    I saw an elderley gentleman today (85 yeas old) who has ruptured his Achilles' tendon (confirmed on ultrasound today) 10 weeks ago. Moderate pain in achilles region & plantar heel. There is a 3-4cm gap between the distal & proximal portions. He is unable to single heel raise.

    What are his options?

    For younger athletes its 50/50 for surgery vs conservative - the outcomes are pretty much the same.

    For the older person.... no idea!

    Thanks Craig. This case would seem not amenable to conservative treatment as suggested in these abstracts, whereby the foot is cast in a plantarflexed position to allow the opposing ends to approximate & heal together.
