    Hi,

    I haven't posted much but I am looking for a bit of advice. Recently, I seen a 20 y.o girl who says her feet feel like they're 'burning' along plantar all the time and bruise/blister very easily and has been the case for the last 5 years at least. When she came in she had a blister on each med heel from new trainers, and bruising along left MLA. She also has a large fibrous HD from previous injury where glass got in her foot, yet she says the HD isn't as painful or uncomfortable as this burning and bruising. She is a student, dosn't play any sports, no change to activity recently, no medical history, no medication. Any suggestions?

    Its a little difficult to comment without a few more specifics on history and examination findings.

    However, a few comments on 'burning' plantar foot pain.

    Generally this is often an indication of neurological pain. ie painful neuropathy. The aetiology may be anything froma compression neuropathy (eg tarsal tunnel syndrome) to a systemic issue (eg hypothyroidism, diabetes, vit B12 deficiency etc). Also there are rarer and more obscure causes that can be haematological/vascular, such as erythromelalgia (associated with essential thrombocythaemia).

    Generally, if the presentation is equal and symmetrical on both feet - think systemic. However, there may be a much more simpler orthopaedic/mechanical explanation - some of your objective findings here would assist (eg she may have a rigid flat foot/tarsal coalition which continually is aggrevating the plantar skin surface when footwear that contacts the area).
