Hi Everyone,

I'm new to the posting on the arena, so please be kind if I'm asking an obvious question.

I have been trying to fine up to date information on the length of time phenol is applied during nail surgery and whether this time is different between a PNA, bilateral PNA and a TNA i.e if it one minute is that per side or in total. Within the organisation I work, there appears to be a vast difference in application from 30 seconds to 4mins.

Hello Pomroo and welcome. I hope that you do not feel anyone on this site might be unkind (unless you are an Aussie posting about the England cricket team, or a Pommie posting on the Australian cricket team!)

I don't think that there is a direct answer to your question since there are so many variables involved. A great deal depends on the practitioner and what is accepted as 'normal' practice locally. My preference is for 3 x 1 minute drying between applications but using a Black's file to abrade the matrical area. Obviously, this has to be done very gently and great care taken not to penetrate the common integument. Some colleagues have told me that they feel this is too aggressive but it works and seems not painful post-operatively. Again, obviously, I spend a little more time with a total ablation, a little less with a PNA. In addition the general health of the patient, peripheral vascular status etc. also has to be taken into account. I suspect that the best bet is to look at the outcomes of your colleagues, draw your own conclusions and see what is most satisfactory from your own point of view.

If the technique used is
completasmente noninvasive ( without cutting the skin ) , you can use long exposure to phenol undamaged and with fewer relapses.
The burned skin protects the caustic action to the deeper tissues.
The population of white skin that burns easily with sun exposure , requires less time.

 

I apply 1+1+1 mins and mop up with guaze in between applications. I too gently work into nail matrix with blacks file. Sometimes i do 2 +1mins if i feel appropriate.
