    2 Cohorts of 70 pts with chronic Achilles Tendinopathy. RCT with custom orthoses vs sham. 12 months f/u. No significant difference between cohorts.

    Main criticism from pods is that 'true' orthoses given didn't include heel raise but authors rebutted this with comment that 'no evidence' heel raises actually work.

    Guess, need to repeat the whole study again but this time with orthoses with heel raise and see what happens then.

    However, this otherwise well designed trial does beg that correcting altered kinematics seen in association with (and likely the cause of) chronic AT doesn't help reverse tendon injury/pain-signalling.

    Ultimately, it's not too surprising since the whole nature of being 'chronic' is that the usual input hasn't worked (and ELE equally isn't the panacea for this). It's at this point that you start sticking needles into people and hitting them with biologics and shockwaves and surgery.
