Effect of Custom Foot Insoles on Postural Stability in Figure Skaters While on Ice

Context: Improvements in postural stability among figure skaters can play a significant role in performance as well as reducing fall-risk.
Objective: The aim of this pilot study was to explore the effect of custom foot insoles on postural stability among advanced figure skaters.
Design: Exploratory Study. Setting: Out of Laboratory. Participants: Nine advanced figure skaters were recruited and 7 completed the study (Age: 38±18.5, BMI: 25±3.6 Kg/m2).
Intervention: Custom foot insoles.
Main Outcome Measures: Primary outcome of changes in postural stability (PS) quantified by center of mass sway with secondary outcomes of ankle and hip joint sway and joint range of motion. Sway measurements were assessed using body-worn sensors while participants wore skates on ice. Postural stability was assessed in single stance as well as during gliding on dominant foot.
Results: A significant improvement in static PS was observed after 6 week use of custom insoles. The center of mass sway reduced significantly on average by 48.44% (p=0.023) and ankle joint sway reduced by 45.7% (p=0.05) during single stance balance measurements. During gliding maneuver non-significant changes were observed for both ankle and knee joint range of motion.
Conclusion: The results of this study suggest proof of concept toward benefits of custom insoles in improving postural stability among advanced figure skaters. In order to generalize the findings randomized controlled trials with larger sample sizes are warranted.

The molded insert can be formed of any moldable material that is substantially firm when cured or dried. The moldable material can be used to fill in the concave portion of the rockered foot bed to form a flat, level or planar surface for the bottom of the wearer's foot, especially in the area corresponding from the beginning of the heel portion of the foot to the beginning of the ball of the foot (e.g., the midfoot portion), thus providing adequate support for the foot's arch during typical ice skating maneuvers.

Epidemiology of Foot and Ankle Injuries in National Collegiate Athletic Association Men’s and Women’s Ice Hockey

Background:
Ice hockey is a high-speed contact sport in which athletes are prone to many different injuries. While past studies have examined overall injury rates in ice hockey, foot and ankle injuries among collegiate ice hockey players have yet to be analyzed.

Purpose/Hypothesis:
The purpose of this study was to elucidate the epidemiology of foot and ankle injuries among collegiate ice hockey players utilizing data from the National Collegiate Athletic Association (NCAA) Injury Surveillance Program. We hypothesized that male ice hockey players would sustain more injuries compared with female ice hockey players and that the injuries sustained would be more severe.

Study Design:
Descriptive epidemiology study.

Methods:
Data on all foot and ankle injuries sustained during the academic years 2004 through 2014 were obtained from the NCAA Injury Surveillance Program. Injury rates, rate ratios (RRs), and injury proportion ratios were reported with 95% CIs.

Results:
Over the study period, the overall rate of foot and ankle injuries for men was higher than that for women (413 vs 103 injuries, respectively; RR, 4.01 [95% CI, 3.23-4.97]). Injury rates were highest during the regular season for both men (358 injuries; RR, 64.78 [95% CI, 58.07-71.49]) and women (89 injuries; RR, 38.37 [95% CI, 30.40-46.35]) compared with the preseason or postseason. The most common injury in men was a foot and/or toe contusion (22.5%), while women most commonly sustained a low ankle sprain (31.1%). For men, foot and/or toe contusions accounted for the most non–time loss (≤24 hours ) and moderate time-loss (2-13 days) injuries, while high ankle sprains accounted for the most severe time-loss (≥14 days) injuries. For women, foot and/or toe contusions accounted for the most non–time loss injuries, low ankle sprains accounted for the most moderate time-loss injuries, and high ankle sprains accounted for the most severe time-loss injuries.

Conclusion:
Foot and ankle injuries were frequent among collegiate ice hockey players during the period studied. For men, contusions were the most commonly diagnosed injury, although high ankle sprains resulted in the most significant time lost. For women, low ankle sprains were the most common and resulted in the most moderate time lost. These findings may direct future injury prevention and guide improvements in ice skate design

Lace Bite: A Review of Tibialis Anterior Tendinopathy in Ice Hockey Players

Ice hockey is a fast‐paced collision sport with a high injury rate. Lower extremity pathology accounts for nearly one third of these injuries. Given the unique nature of the sport, there are various injuries seen more commonly in ice hockey players, including “late bite” (or “skate bite”). Lace bite is an inflammation of the tibialis anterior tendon which often affects hockey players in the beginnings of the season. This is often caused by irritation of the tibialis anterior from the tongues of skates which have not yet been broken in. These patients often present with edema, tenderness over the anterior tibial tendon, and pain with dorsiflexion. Treatment of lace bite involves skate modifications such as breaking in the skate before increased play, and padding over the anterior ankle to reduce pressure from the tongue. Topical and systemic anti‐inflammatories, along with icing and activity modification are also helpful in reducing symptoms. These injuries are often short‐lived and resolve within two weeks. By having a better understanding of this injury, team physicians will be able to more promptly diagnose and effectively treat this condition.

Injuries and Overuse Syndromes in Rink Hockey Players

Rink Hockey is a high-speed low-contact sport with a widely recognized injury potential. Played with a hard ball by players wearing little or no protective gear, the rates of concussions and head injuries are of high interest. In this study, we reviewed and investigated injuries sustained by 108 German National League rink hockey players. We conducted an epidemiological cross-sectional study to determine sport-specific injuries and injury patterns in female and male rink hockey players. Data were collected by a standardized questionnaire. A total of 108 players participated in the study. The combined rate of injuries were 9.4/1000 athlete exposures. There was no significant difference between injury rates during games and practice. A sex-specific difference was not detected. Head injuries were the most frequent type (20.8% of all injuries). Concussions made up 7% of all head injuries with a rate of 0.76/1000 athlete exposure. Ball contact was the cause for 31% of injuries, while player contact generated 26.2% of all injuries. High rates of ball-contact-related injuries resulted in time loss and medical consultations. Head injuries were frequent, however did not result in significant time loss as compared to other injuries.

Background:
Orthopaedic injuries are common in ice hockey at all levels and can result in physical and psychological adverse effects on these athletes.

Purpose:
Primarily, to summarize published data on orthopaedic hockey injuries at the junior through professional level. Secondarily, to characterize the literature based on anatomic site injured, return-to-play rates, cause/mechanism of injury, time lost, and treatments used.

Study Design:
Scoping review; Level of evidence, 4.

Methods:
PubMed, EMBASE, Cochrane library, and SCOPUS were searched using the terms “hockey” and “injuries” using PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, and 4163 studies involving orthopaedic injuries were identified. Our inclusion criteria consisted of accessible full-text articles that evaluated orthopaedic injuries in men’s ice hockey athletes of all levels. We excluded case reports and articles evaluating women’s ice hockey injuries, as well as those evaluating nonorthopaedic injuries, such as concussions; traumatic brain injuries; and facial, dental, and vascular injuries, among others. Studies were divided based on level of play and anatomic site of injury. Level of evidence, year published, country of corresponding author, method of data collection, incidence of injury per athlete-exposure, and time lost were extracted from each article.

Results:
A total of 92 articles met the inclusion criteria and were performed between 1975 and 2020, with the majority published between 2015 and 2020. These were divided into 8 anatomic sites: nonanatomic-specific (37%), intra-articular hip (20.7%), shoulder (9.8%), knee (8.7%), trunk/pelvis (7.6%), spine (7.6%), foot/ankle (6.5%), and hand/wrist (2.2%). Of these studies, 71% were level 4 evidence. Data were obtained mostly via surveillance programs and searches of publicly available information (eg, injury reports, player profiles, and press releases).

Conclusion:
This scoping review provides men’s hockey players and physicians taking care of elite ice hockey athletes of all levels with a single source of the most current literature regarding orthopaedic injuries. Most research focused on nonanatomic-specific injuries, intra-articular hip injuries, knee injuries, and shoulder injuries, with the majority having level 4 evidence.
