    42 year old patient (me) with 10 day history of right hip pain. I run approx 3 times a week for maybe 5 km & pain commenced when running. Resolved with rest but last few runs has persisted after finishing run. Saw a Physio for the pain & he was of no benefit (did lots of testing but couldnt identify location of pain therefore no diagnosis). I will make another appointment with a different Physio who maybe has better knowledge of hip/sports injuries but what could the structure be? I can reproduce the pain with a single leg squat & bending at my waist to the left hand side. What structure ('s) am I straining with this movement?

    which part of your hip is hurting? front/back/side...
    have you always been a runner or have you just taken it up?
    Have you changed distance or where you run?
    Any trauma or sound at the time it started to hurt?

    Structures/possibilities/hypotheticals...
    posterior - weakness in gluteals
    lateral - trochanter bursitis, ITB
    anterior - quads, hip flexors

    I have been a recreational runner for quite a few years & have averaged 15-20km/wk for some time. It's probably not the place to post the query but I thought after yesterdays appointment that cost me $90 for little to no benefit, Id at least see what Pods on here thought. There was no trauma, no change of distance or running route.

    I can create the pain with a single leg squat & then bending to the opposite side. Pain is in the lateral hip region (trochanter) & can extend into glutes slightly.

    Trochenteric bursitis / glute med reactive tendonopathy??
    Tender to palpate posterior greater trochanter of hip??
    More painful on UP stairs or UP hill? & possibly positive FABERS

    Sounds like you have a quadratus lumborum trigger point with referral to the lateral hip. This is sometimes known as pseudo-sciatica. When you are in single limb sqat, the quadratus is used to maintain side balance. When stretched while active, the pain referral reproduces your symptoms. Since the painful site is not in and of itself painful (hence the negative physical exam findings) but instead is the site to which pain is referred. Treatment is to have Tp type therapy on the quadradus lumborum and to stretch it as well. You can also try acupuncture to relieve the Tp. Good luck....the pain from this can be quite intense.

    If the piriformis has been pulling the greater trochanter left to keep more weight over the right foot then bending left would stretch it against CNS preset. Phisio needs to look for the cause of the weight shift if you can not identify it yourself.
