An interesting article in LER on Managing turf toe in football players. Grade 1 and 2 turf toe hyperextension injuries have good outcomes when treated non-operatively. The optimal treatment for grade 3 turf toe injuries with complete capsular avulsion, however, is unclear.

Many patients with turf toe do very well with a steel insole, either a full length or 1/2 length. It stabilizes the forefoot to allow the IPJ to heal.

I have come to see turf toe and hallux limitus as conditions which hurt because the MTP joint does NOT move when demand is the greatest. Looking at this as a problem which hurts because is moves misses the underlying issue. The most common issue is inhibition of the peroneus longus, and secondary failure to stabilize the joint during max demand. Manipulation of the ankle and cuboid and perhaps the 1st metatarsal cuneiform joint can resolve this spontaneously. I have treated this with success countless times. Using steel plates only serves to invert the foot and create symptoms elsewhere in the body.
