The Mau-Reverdin Osteotomy: A Short-Term Retrospective Analysis

We reviewed 33 consecutive Mau-Reverdin osteotomies in 23 patients performed for correction of hallux abducto valgus from November 2010 to May 2013. All patients were followed up and evaluated for a mean of 401 days and median of 360 days after surgery. In each foot, the preoperative first intermetatarsal angle, hallux abductus angle, and proximal articular set angle were obtained. The mean correction of these angles was as follows: intermetatarsal angle 10.5? ? 3.31?, hallux abductus angle 24.4? ? 8.8?, and proximal articular set angle 28.39? ? 11.2?. Furthermore, we evaluated for metatarsus elevates, and no statistically significant first metatarsal elevation was present in any of the 33 feet (p < .0001). Additionally, 21 of the 33 feet (63.6%) were available for first metatarsophalangeal joint American Orthopaedic Foot and Ankle Society scale score evaluation. The mean preoperative score was 25.5 ? 16.7. After correction, the mean American Orthopaedic Foot and Ankle Society scale score had increased to 95.4 ? 5.7. All these differences were statistically significant (p < .0001), and the patients had a very high level of satisfaction. In all 33 feet, no deep infection, malunion, nonunion, avascular necrosis of the first metatarsal, or hardware failure developed. One patient developed hallux varus deformity. The Mau-Reverdin osteotomy is a very effective and reproducible procedure that successfully corrects large bunion deformities and provides patients with a high level of satisfaction and a low complication rate.

Percutaneous Hallux Valgus Correction: Modified Reverdin-Isham Osteotomy, Preliminary Results.

BACKGROUND:
Hallux valgus (HV) is a metatarsophalangeal joint deformity that can be classified as mild, moderate, or severe. Treatment is recommended for pain or severe deformities. Recently, operative percutaneous correction techniques have been performed to treat mild deformities.

MATERIALS AND METHODS:
A retrospective, single-surgeon, single-center study of 49 HV percutaneous correction using a modified Reverdin-Isham osteotomy was conducted. HV, intermetatarsal angle (IM), proximal articular set angles (PASA), and American foot and ankle functional score (AOFAS) were assessed pre- and postoperatively by a single operator. Statistical analysis was performed using a Wilcoxon rank test. Medium time of follow-up was 34 months.

RESULTS:
HV mean value decreased from a preoperative medium value of 35.18° to 14.3° postoperatively, IM mean value decreased from 15.5° to 8.7°, and PASA from 7.2° to 5.25°(p<0.001 for HV and IM, p<0.125 for PASA reduction). Postoperatively AOFAS medium score was 95.

DISCUSSION:
In our series, functional and clinical results of percutaneous osteotomy without osteosynthesis were comparable to other percutaneous and conventional techniques, both in clinical and radiological findings. High level of patient's satisfaction and improvement on pain-related symptoms are even better referred to traditional techniques. Modified Reverdin-Isham osteotomy technique differs from the others for translation of metatarsal head after osteotomy; we do not correct PASA angles, but we can obtain more HV and IM correction and include some severe HV. Our results suggest that translation of metatarsal head could give higher HV angle correction.

Background
In the last decade, interests in minimal incision surgery have been growing. Theoretically, this kind of procedure could decrease time to recovery and rehabilitation, with a reduction in morbidity. The present study assessed clinical and radiological outcomes and complications of percutaneous surgery for mild-to-moderate hallux valgus using Reverdin–Isham and Akin osteotomies without fixation after 60 months of follow-up.

Methods
A series of 48 patients (57 cases) with medium-to-moderate hallux valgus underwent the same percutaneous surgery, between 2003 and 2011. Data collection involved preoperative dorsal flexion, plantar flexion, M1P1, M1M2, DMAA angles, AOFAS scale score, and subjective satisfaction.

Results
AOFAS scale score rose from a preoperative median of 55.9–89.2/100 postoperatively (p < 0.001); 51 surgical procedures (89.5%) were considered as satisfactory or very satisfactory by patients at the end of follow-up. Hallux valgus and distal metatarsal articular angle (DMAA) were significantly reduced (29.3° and 14.1°–15.4° and 7.7°, p < 0.001, respectively). There was a significant increase in MTPJ 1 stiffness (p < 0.001).

Discussion
Percutaneous correction by Reverdin–Isham and Akin osteotomies seems to be effective in isolated medium-to-moderate hallux valgus. Stiffness observed is comparable to other percutaneous and open procedures but needs to be compared in a randomized controlled clinical trial to extra-articular percutaneous procedures without capsule detachment in association with an internal fixation which allows an early mobilization.

Early Functional and Radiological Results of Mau Osteotomy for Moderate-Severe Hallux Valgus Deformity

INTRODUCTION: The aim of the present study was to evaluate the early functional and radiological results of Mau osteotomy in the treatment of moderate -severe hallux valgus deformity.
METHODS: A retrospective evaluation was made of 16 patients undergoing Mau osteotomy for treatment of hallux valgus deformity between July 2016 and January 2019. Clinical findings of patients were evaluated in preoperative and the last follow-up visits using the American Orthopaedic Association Foot and Ankle Score (AOFAS) and Visual Analog Scale (VAS). Radiological findings were measured with hallux valgus angle (HVA) and the intermetatarsal angle (IMA) on anteroposterior and lateral standing radiographs of both feet.
RESULTS: 9 (56%) patients were female and 7 (44%) male with a mean age of 43.7 years. The mean follow-up period was 18 months (range, 6-27 months). A statistically significant improvement was determined in AOFAS and VAS postoperatively compared to the preoperative values (p<0.05). Radiologically, the HVA improved from 43.75 ± 7.58 preoperatively to 22.13 ± 6.63 at the final follow-up examination, and the IMA from 16.00 ± 2.82 to 7.44 ± 3.38 (p<0.05). Excellent and good results were obtained in 81% of the patients.
DISCUSSION AND CONCLUSION: In conclusion, Mau osteotomy can be considered an effective and reliable method in the treatment of hallux valgus deformity, based on the excellent and good clinical and radiological results obtained in 81% of the current study patients.
