I first suffered from Iliotibial band syndrome in 1975 or 1976. At the time it was addressed as lateral knee pain. It responded in my case with a heel lift on the ipsilateral side. When I was at the Cleveland Foot Clinic in the Sports Medicine department, I would supervise the treatment of 40 runners per day. In most of the cases of IT Band syndrome responded to a 1/4" heel lift on the ipsilateral side. There are really two conditions that occur there which is a friction syndrome and a tension syndrome. The cases that did not respond had an extreme amount of tightness of the Tensor fascia lata.
