    Hi All. I think I need to update my form that patients fill in prior to their first appointment. Does anyone have one they are especially proud of to share? Thanks

    Off topic for this thread, but have a read of the article linked in this thread: Why do ineffective treatments seem helpful? on why people may perceive an improvement at each visit when there really isn't - its an enlightening read and well worth thinking about.
