hi,
never used this site to ask advice before so please forgive any foibles.
i have a patient in their mid 20's. very active playing sports most days.
they have a rigid flat foot with approximately 12mm of a leg length difference on the affected foot side.
any advice on ways this could be helped would be appreciated.
when i cast them it was with the foam box with no attempt to put the foot into subtalar neutral.

Hi Wayne,

If by rigid you mean no movement then you will not be able to put them in "STJ neutral".

So with a true rigid foot I would address the leg length with a full foot lift to balance the hips or shoe mods and get then into a very cushioning device.

If by Rigid you mean pronated to end range of movement I would suggest you look up posts about medially deviated stj axis and you will find much more info which can help.

What is the reason for needing to treat? You said the patient was very active/sporty but you don't mention any particular pain or symptoms.

 

sorry about that. client does have knee pain and lower back pain which has associated with the condition.

Bilateral knee pain? Medial, lateral, anterior? What makes you think it is related to his foot posture or leg length? Can you tell us what anatomical structures are painful, during what movement or activity etc? How long has the patient had symptoms? What has led to a "leg length difference"? Congenital, acquired/surgical? By the way - impressed at your precision of measurement - how did you manage to narrow it down to 12mm exactly?! Sorry for so many questions but it is impossible to us to try to help without more information.

patient described the knee pain as generalised. had the flat foot all his life.
as to being so precise with my measurement 12mm is close enough to the 1/2 inch that it is in british measurements
