    Hi Guys,
    I have a patient who had a forced dorsiflexion iinjury which has been imaged by MR to show chondral crush injuries at Navicular/medial cunieform and base of 1 and 2 mets. Injury occurred 6 weeks ago, initial presentation at week 5. He cannot use per longus at all because of pain but he has been walking albeit badly.
    I am fishing for thoughts on the value of a good Vacoped cam walker for a while even at this time. He is reluctant to do much, even getting into some stiff shoes has been a battle. He was hoping to be surfing again very soon but I think the MRI proof of genuine damage may have sunk in. Your thoughts please

  2. A walking boot brace is a must at this time and will probably be needed, considering the pain and MRI findings, at least for 2-4 weeks just to let the foot to heal sufficiently to allow more normal walking, let alone sports activities.

    I don't really ask patients what they want. Rather I tell them what they need to do to get better after having a full conversation with them on what their activities include and what their expectations are. If they then say they don't want to do that treatment then I tell them that it is their right to deny my medical advice and continue being in pain and continue having disability. It's their choice.
