Aneurysms of the foot arteries are uncommon but can lead to devastating complications such as acute foot ischemia or arterial rupture if left untreated. In this case series, we present four cases of aneurysms of the foot: one true dorsalis pedis artery aneurysm and three cases of post-traumatic plantar artery pseudoaneurysms with arteriovenous fistulas. All four patients were successfully managed with surgical excision of the aneurysm with or without arteriovenous fistulas ligation. Our case series is followed by discussion on the etiology, clinical presentation and management strategy of patients with aneurysms of the foot arteries.

Aneurysms of the foot arteries are rare. A case of a true dorsalis pedis artery aneurysm in a 69-year-old man, with no history of local trauma or injury on his foot, is presented. Clinical examination and ultrasonic imaging confirmed the aneurysm. Dorsalis pedis artery aneurysm was resected and an end to end anastomosis was performed, without complications. Arterial duplex scan findings, operative procedure and literature review are presented and discussed.

Background: Reports of true aneurysms of the lower leg are rare. Among them, cases involving young patients are all the more rare, and there are many unexplored aspects to this pathological condition.

Case presentation: This is a case of a 30-year-old woman who was referred by an orthopedic surgeon with a chief complaint of severe pain during walking and landing. Angiography revealed multiple aneurysms and arteriovenous fistulas in the posterior tibial artery and lateral plantar artery. We anastomosed the lateral plantar artery and interposed the posterior tibial artery using the great saphenous vein.

Conclusions: Although aneurysms in the arteries of the feet are rare, multiple true arterial aneurysms were observed in the lower leg of a juvenile patient in the present case. Dilation of arteries other than the aneurysm was also observed, suggesting that arteriovenous fistula and arterial occlusion may have been the causes of the true aneurysm of the lower leg. This is a valuable finding, suggesting a cause of aneurysm other than age-related atherosclerotic changes.

Objectives: Aneurysm of the pedal arteries is uncommon. I present a case of non-traumatic fusiform true aneurysm of the dorsalis pedis artery in an otherwise well 45-year-old man. Color flow duplex imaging revealed aneurysmal dilation, involving all layers of the artery wall, measuring 16.5 * 10 mm with irregular intraluminal thrombus across a 6.33-mm segment. Due to concerns over embolization, our patient underwent successful ligation of the dorsalis pedis artery. He had an uneventful post-operative recovery.

Methods: Case report.

Results: No postoperative complication or signs of ischemia.

Conclusions: Treatment of asymptomatic dorsalis pedis artery aneurysm may be of value to prevent risk of thrombo-embolic complications, foot ischemia, or rupture without warning signs. Patency of the pedal arch is important to avoid foot ischemia in case of dorsalis pedis artery ligation.
