To wet peoples appetites, here are the abstracts of my presentations at this upcoming conference:

Foot Orthotics for Athletes
Overuse injuries affect 30-70% of runners in any one year. Traditionally, a number of factors are involved in these injuries that include the running footwear used and issues with them; the training routine; overtraining; the particular running technique or form used by the runner; muscles strength imbalances and flexibility issues; and issues with foot and lower limb biomechanics or alignment; and issues with the proximal control of lower limb biomechanics. Interventions, including foot orthoses are traditionally used to address these issues. The use of the foot orthoses was based on the assumption that problems with foot biomechanics was an underlying a factor in a number of the injuries. While the evidence for this abnormal biomechanics is not strong and in many cases contradictory, the outcome studies have clearly demonstrated the efficacy of foot orthotics in runners as an intervention when aimed at attempting to change the alignment or posture of the foot. More recently there has been increased interest in the less use of foot orthoses and barefoot or minimalist running. As a result there is now greater appreciation of the role of tissue tolerance to load. Overuse injuries essentially result when the load on the tissue is above what the tissues can tolerate. As a result of this changing focus, the most recent advocated approach to the treatment of overuse injuries in runners is to focus on the use of foot orthoses and running technique modification to reduce to the load on the injured tissue as opposed to the traditional approach to changing alignment or posture. Attention is also focused on the ability of the tissues to take the load and interventions used to increase tissue tolerance. The foot orthoses are then generally considered short or long term interventions depending on the magnitude of the forces creating the tissue damage and if the tissues can be adapted to the load. The evidence for these approaches will be reviewed.

Clinical Testing for Foot Orthotics Prescription
Foot orthoses are widely used to treat a wide range of lower limb and foot problems. There is also a wide range of foot orthoses and a wide range of additions or modifications to that wide range of foot orthoses. While there is some limited research on some of the types of foot orthotics and some of the modifications, there is very little evidence on them all, let alone clear agreed upon clinical guidelines. Within all that variety of types of foot orthoses and their infinite possible variations, there cannot possible be a ‘one best foot orthotic’. The advocated approach is to consider the best foot orthotic as the one that has the design features to meet the prescription variables for the one individual patient. To achieve this, all the potential different foot orthotic design features need to be documented. Each of the design features will then have clinical indications that will come from clinical testing and clinical observation. The collection of prescription variables from this clinical process are then collated to arrive at the design features that are needed to be delivered by the foot orthotic to achieve the desired clinical outcomes. The appropriate foot orthotic for that patient is the one that can deliver all those design features.

Issues in Diabetic Foot Orthoses
Plantar ulcers in the neuropathic foot are a significant contributor to the morbidity and quality of life in those with diabetes. The association of the sensorimotor neuropathy and the subsequent biomechanical alterations increase the plantar pressures that are the main risk factor for foot ulcers. A number of therapeutic interventions such as footwear, casting, padding, gait retraining, biofeedback and foot orthotics have been investigated for their plantar pressure reduction capabilities. Some clinical trials have also investigated the efficacy of different methods. The actual pathomechanical mechanism of plantar foot ulcers has not been clearly elucidated but is related to a pressure induced autolysis of the tissues; a potential transient ischemia from the pressure; and also a potential tearing of the tissues from the shearing stress. The design features incorporated into foot orthoses for the prevention of plantar foot ulcers and lowering the risk of the high recurrence rate of foot ulcers need to take into account these pathomechanical mechanisms. Different design features achieve the desired outcomes.

This year's conference is looking quite good, the full timetable has been released - see attached. I understand registrations can be taken on the door too! At £80 plus VAT for the weekend, it's the cheapest 3 day conference in P&O.
