Care pathway to save the ischaemic diabetic foot

    This is an excellent study which confirms many of our beliefs regarding the treatment of diabetic patients, in that multidisciplinary teams give better outcomes.

    Before this study many of us were working towards this, and had anicdotal evidence to show results.

    From this it becomes obvious that ischemia is the critical factor. vascular interventions are costly and are not always helpful since healing depends on many factors. Pneumatic circulation boosters have been shown to be helpful http://1.usa.gov/QX4At9 .Pneumatic compression requires being tethered to a compressor for several hours daily. Since the portable battery powered venowave is easier to use similar and possibly better results will occur. Preliminary small scale study had 8/8 patients healing in less than 30 days. Larger scale studies are being planned. Since it has been FDA, Health Canada and CE approved for circulation issues there are few reasons not to use it. If patients can't afford it the company will provide free units if the attending physician is willing to take pictures and record the outcomes. You can look up the product on line and find all the information required there.

    While a multidisciplinary team may be more effective, it will only be as good as the technology employed by its members. If one wants to do more with less invasive techniques using a mechanical circulation booster, such as our Venowave will make things heal faster. Clinicians should be advising their patients about FDA approved circulation boosters which have been cliniclly proven to increase circulation. Many if not all amputations would be prevented by using such a simple yet effective device.
