In-shoe multi-segment foot kinematics measurement during the stance phase of running using a stretch strain sensor

Multi-segment foot kinematics during shod running are difficult to investigate in clinical settings. Stretch strain sensors can measure foot kinematics; however, whether they can evaluate foot kinematics during shod running or at the midfoot kinematics remains unclear. The aim of this study was to investigate the stretch strain sensor could reveal differences between shod and barefoot conditions and midfoot kinematics during running. Eighteen healthy adults were included in the study. A stretch strain sensor and three-dimensional motion capture system were used to measure foot kinematics during barefoot and shod running with a rearfoot strike pattern. The correlation between the amplitudes of the two signals during barefoot running was investigated, and the similarity between the two signals was evaluated using the cross-correlation coefficient. Statistical parametric mapping was used to compare shod and barefoot conditions. Shod running had significantly lower sensor strain from 30 % to 100 % stance compared to barefoot running (p < 0.05). The sensor amplitude was significantly correlated with the shank-rearfoot frontal (r = 0.668, p = 0.002), the rearfoot-midfoot transverse (r = 0.546, p = 0.02), and the midfoot-forefoot sagittal planes (r = 0.563, p = 0.01). A high cross-correlation was observed between the sensor signal and the shank-rearfoot sagittal, frontal, and transverse planes and the midfoot-forefoot sagittal plane. Development of an IMU based 2-segment foot model for an applicable medical gait analysis

Background: The two most commonly instrumented gait analysis tools used are Optical Motion Capture systems (OMC) and Inertial Measurement Units (IMU). To date, OMC based gait analysis is considered the gold-standard. Still, it is space-, cost-, and time-intense. On the other hand IMU systems are more cost- and time effective but simulate the whole foot as a single segment. To get a more detailed model of the foot and ankle, a new 2-segment foot model using IMU was developed, comparable to the multi-segment foot models assessed by OMC.

Research question: Can an IMU based 2-segment foot model be developed to provide a more detailed representation of the foot and ankle kinematics?

Methods: To establish a 2-segment foot model, in addition to the previous 1-segment foot model an IMU sensor was added to the calcaneus. This allowed the differentiation between the hindfoot and forefoot kinematics. 30 healthy individuals (mean age 27 ± 7 years) were recruited to create a norm data set of a healthy cohort. Moreover, the kinematic data of the 2-segment foot model were compared to those of the traditional 1-segment foot model using statistical parametric mapping.

Results: The 2-segment foot model proved to be applicable. Furthermore, it allowed for a more detailed representation of the foot and ankle joints, similar to other multi-segment foot model. The healthy cohort's norm data set showed a homogeneous motion pattern for gait.

Conclusion: The 2-segment foot model allows for an extension of IMU-based gait analysis. Futures studies must prove the reliability and validity of the 2-segment foot model in healthy and pathologic situations.

Understanding the design differences between multisegmented foot models and their impact on joint kinetic outcomes

Multisegmented foot models (MSFMs) are used to capture data of specific regions of the foot instead of representing the foot as a single, rigid segment. It has been documented that different MSFMs do not yield the same joint kinematic data, but there is little information available regarding their use for kinetic analysis. We compared the moment and power at the tibiotalar, midtarsal, and metatarsophalangeal joints of four MSFMs using motion capture data of young adult runners during stance phase of barefoot walking and jogging. Of these models, three were previously validated: the Oxford, Milwaukee, and Ghent Foot Models. One model was developed based upon literature review of existing models: the "Vogel" model. We performed statistical parametric mapping comparing joint measurements from each model to the corresponding results from the Oxford model, the most heavily studied MSFM. We found that the Oxford, Milwaukee, Vogel, and Ghent Foot Models do not provide the same kinetic results. The differences in segment definitions impact the degrees of freedom in a manner that alters the measured kinematic function of the foot, which in turn impacts the kinetic results. The results of this study capture the variability in performance of MSFMs as it relates to kinetic outcomes and emphasize a need to remain aware of model differences when interpreting results.

The understanding of foot and ankle biomechanics is improving as new technology provides more detailed information about the motion of foot and ankle bones with biplane fluoroscopy, as well as the ability to analyze the hindfoot under weightbearing conditions with weightbearing computed tomography. Three-dimensional anatomical coordinate systems are necessary to describe the 3D alignment and kinematics of the foot and ankle. The lack of standard coordinate systems across research study sites can significantly alter experimental data analyses used for pre-surgical evaluation and post-operative outcome assessments. Clinical treatment paradigms are changing based on the expanding knowledge of complex pes planovalgus morphologies or progressive collapsing foot deformity, which is present in both neurologic and non-neurologic populations. Four patient cohorts were created from 10 flexible PCFD, 10 rigid PCFD, 10 adult cerebral palsy, and 10 asymptomatic control patients. Six coordinate systems were tested on both the talus and calcaneus for all groups. The aim of this study was to evaluate axes definitions for the subtalar joint across four different patient populations to determine the influence of morphology on the implementation of previously defined coordinate systems. Different morphologic presentations from various pathologies have a substantial impact on coordinate system definitions, given that numerous axes definitions are defined through geometric fits or manual landmark selection. Automated coordinate systems that align with clinically relevant anatomic planes are preferred. Principal component axes are automatic, but do not align with clinically relevant planes and should not be used for such analysis where anatomic planes are critical.
