My wife is studying for her Racgp exam and occassionally asks me some of her foot and ankle questions. On of them was an MCQ and I got it wrong. Please try yourself and post your answers. I will post the correct answer in one week.
What is the most common cause of arthritis in the foot and ankle?
a) Gout
b) Osteoarthritis
c) Psoriatic Arthritis
d) Rheumatoid Arthritis

b) Overuse / interarticular trauma (causing Osteoarthritis)
c) Auto Immune (causing Psoriatic Arthritis)

Sorry all. I have located the exact question . Sorry to be misleading. Now that I read it again it seems more obvious. Here it goes:
A 35 year old man presents with recent onset of pain and tenderness in his left ankle.
The most common type of arthritis to affect the foot and ankle is:
a) gout
b) osteoarthritis
c) psoriatic arthritis
d) rheumatoid arthritis
e) Ankylosing Spondylitis

mmm...badly written question, not really enough info.
But most common cause in a 35 year old male?
Could the age and gender be a hint pointing towards AS? -
I assumed the question meant the most common cause of arthritis in that younger age group. If that is the case I would have suggested AS rather than OA.

I thought age may be relevant but the question wording is so poor you could make many assumptions regarding the examiner's intention. If in doubt with MCQ, choose b).

However, now that we have some more info...
"35 yr old male with recent onset of pain in left Ankle"
Now, I'm leaning more towards Gout or possibly Psoriatic Arthritis.
The age is good for AS, but not most common site in the ankle.
The sudden onset can be found with both Gout attacks and Psoriatic...
I'm going with Gout.
Here's hoping our 35 yr old male is a raging alcoholic, who likes eating sardines and offal... -

Well before you corrected the question I would have said Osteo caused by Trauma....footy/bikes/skiing etc. But thinking about it AS would cause a change in gait but without more history it is impossible to say in this case.
I think its a catch question....the facts about the patient are irrelivant, they are just asking for "the most common type of arthritis to affect the foot and ankle is......"
Therefore the answer is OA.... -

Now my immediate reaction was to tell my wife it was a mistake.
It is an updated online MCQ resource supplied to RACGP members.
However I couldn't locate evidence to back up OR refute my answer.
My answer was OA.

Well, the question asks "what is the most common CAUSE..."
It does not ask what is most common TYPE?
The most common CAUSE of arthritis in the foot ankle is GRAVITY!
